Job Summary

Position develops, implements, and monitors clinical pharmacy programs to improve patient care with an additional focus on decreasing cost; provides clinical services to patients and professional staff; interacts with medical staff on drug utilization and cost containment. May be asked to assist in supervising the activities of all department employees in consultation with the department Director and/or Pharmacy Manager.Job Description

Minimum Qualifications:

Education: Pharm D. degree or clinically oriented M.S. degree and licensure in the state of Texas or BS degree with equivalent experience. Preferred completion of ASHP accredited pharmacy practice residency or fellowship or demonstration of equivalent experience.

Licenses/Certifications: Pharmacy Sterile Product certification through an in-house program or other accredited program if appropriate.

  • Knowledge of pharmacy and hospital computer systems and computer-assisted devices.

Principle Accountabilities:

  • Develops and maintains a communication system which assures effective flow of information to and from staff. Serves as a role model for competent professional practice by educating, mentoring, and supporting pharmacy students, staff, residents, and peers.

  • Seeks and recommends potential opportunities for cost containment.

  • Seeks new ways of improving clinical pharmacy services. Works with others to continually identify areas for improvement.

  • Effectively plans and manages resources for continuity of care between the patient care area, Pharmacy and other settings. Participates in orientation and continuing education programs. Serves as a resource to pharmacy staff, physicians, and nurses.

  • Assists with clinical research designed to evaluate clinical and economic outcomes of drugs and drug regimens.

  • Works to develop, coordinate, and evaluate target drug programs, restricted drug programs, medication use-based performance assessment and improvement indicators, drug usage evaluation programs and cost containment programs. Develops and implements utilization reviews targeted for expensive, potentially harmful or improperly used drugs.

  • Provides direction and guidance for development and review of clinical pharmacy educational services. Assists with implementation of educational programs for physicians and nursing staff.

  • Ensures safe care to patients, adhering to policies, procedures, and standards, within budgetary specifications, including time management, supply management, productivity, and accuracy of practice.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff. Oversees the clinical students/residency program.

Job Summary Position responsible for ensuring clinical excellence in either an assigned work group or for an assigned patient population. Responsible for appropriate supervision of assigned personnel and performing direct patient care in a cost-effective manner. Serves a resource and mentor for the staff, students and volunteers while assisting in setting up educational programs, provide input for program development, and serves as a resource for community education and marketing. Must be able to demonstrate knowledge and advanced skills necessary to provide care appropriate of the patient served.Job Description Location: Greater Heights Shift/Schedule: 10am-6:30pm Monday-Friday (1 weekend per month) Minimum Qualifications: Education: Bachelor's or graduate degree in occupational therapy from an accredited university-based occupational therapy program. Licenses/Certifications: Texas Board of Occupational Therapy Examiners license. Experience / Knowledge / Skills: * At least five (5) years of clinical experience required. Demonstrates clinical excellence using the following criteria: * Demonstrated ability and willingness to mentor/train staff or student interns. * Provides clinical leadership and demonstrates expertise for a particular patient type/group. * Enhances an existing or new clinical program for a patient type/group. * Actively participates in a hospital and/or system or departmental committee/task force. * Designs and implements original staff development program which facilitates the clinical competencies of other staff members. * Initiates and/or provides leadership in an ongoing departmental interest/study group. * Demonstrates leadership skills by assisting Director or Manager with daily operations including but not limited to performance appraisals, performance issues, scheduling, marketing, interviewing, and training of staff, payroll, and cost management. (2 points) * Plays an active role in a clinically relevant research project outside requirement for degree. * Obtains certification in a relevant treatment technique that enhances current scope of practice. * Obtains national certification/licensure in a specialty field by a national organization, such as AOTA, APTA, ASHT. * Obtain advanced master's degree in clinically relevant area. * The position requires occasional/frequent lifting up to 75-100 pounds and carrying of up to 30 pounds; prolonged standing; frequent walking, stooping and squatting. Fine hand dexterity, eye-hand coordination, the ability to receive oral and written communication, and to communicate verbally and in writing are also necessary. Accurately interprets evaluation findings. Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals. Implements an occupational therapy treatment program and provides advanced occupational therapy treatments. Demonstrates competency in performing advanced occupational therapy skills. Assesses patients periodically to monitor progress towards treatment goals, and alters goals, plan or program as indicated for assessment. Motivates and instructs patient/caregiver using appropriate methods. * Communicates effectively with staff, patients/caregivers and professional colleagues to include physicians and insurance companies. Manages time effectively. Documents appropriately in the medical record and completes other documentation duties accurately and efficiently. * Serves as clinical resource to staff, hospital, schools and the community in general. Acts as clinical instructor to OT and OTA interns. Supervises assigned staff. Actively participates in organized community service/functions as a representative of the profession. * Identifies need for new departmental programs, or for revision of existing ones in response to the changing health environment.
