The Clinical Pharmacist Specialist provides comprehensive pharmaceutical care in a specific area of specialty through integrated patient assessment and monitoring, standardization of drug distribution, clinical service, teaching, and pharmacy representation among the medical and nursing staff. Clinical Pharmacist Specialists perform all expectations of the Staff Pharmacists, in addition to expectations specific to Clinical Specialists.

1.B.S. or Pharm. D. from an ACPE-accredited school of pharmacy required
2.Substantial experience in area of specialty or completion of an ASHP-accredited residency program in area of specialty
3.Currently licensed in good standing to practice pharmacy in the state of Virginia.
4.Board certification is highly desirable.
5.Must maintain competency in unique aspects of drug therapy for patients in area of specialty.
6.2 -4 years of hospital experience preferred, with strong clinical pharmacy background required.
7.Must demonstrate professional and clinical knowledge of current drug therapy.
8.Ability to provide clinical leadership and education

The Clinical Pharmacist of Quality and Care Management will play a critical role in developing and leading the execution of value-based strategy for pharmacy cost & utilization and quality metrics. The Clinical Pharmacist will serve as a subject matter expert in drug information and therapy to nursing staff, allied health professionals, Clinically Integrated Network (CIN) providers and patients. This position will also assist in the development and implementation of programs and processes to improve patients' understanding of their medications and medication administration and the quality and efficiency of medication management across the CIN. * Responsible for managing and implementing clinical pharmacy programs to promote quality and cost-effective drug utilization. * Partner with Director of Quality and other clinical leaders to identify and develop Pharmacy related population health programs to improve formulary compliance, medication adherence, reduce excess and inappropriate prescribing and promote value based pharmacy programming with our CIN partners. * Serves as resource to CIN and ACO providers, care managers, and other healthcare professionals to provide medication reviews and to assist in medication reconciliation activities for high-risk patients. * Develop customer and physician engagement programs to ensure appropriate use and improve health outcomes * Partner with the CIN & ACO Clinical support team to develop and deliver population health programs related to pharmacy medication adherence and quality measures. * Provides oversight of cost-effective use of medications and treatments. This includes risk reduction and correct utilization of resources to optimize care. * Identify barriers to medication adherence and provide solutions to enable patient success. * Perform analytics to identify and evaluate opportunities for cost savings and quality improvement and develop clinical programming that can assist our ACOs in improved pharmacy management, including but not limited to new reports, provider and patient education material, partnerships with specialty pharmacies and infusion sites, etc. * Collaborate with business analysts to develop and enhance pharmacy reporting. * Provide pharmacy trend/spend insights on underlying drivers by integrating clinical knowledge/experience and analytical skills. * Present ACO Specific pharmacy related data, clinical findings and literature to senior leadership, as well as external and internal teams. * Works collaboratively with cross functional teams and a broad set of matrix partners to ensure timely execution of business objectives. Education and Experience: * Registered pharmacist required. * Pharmacy residency experience in managed care preferred. * Broad-based comprehensive knowledge related to pharmacy trends, population health programming and project improvement. * 3 years' experience in patient care and working directly with clinicians or physician practices. * Experience with pharmacy data and analytics preferred. * Strong presentation skills preferred. * Minimum 3 years of managed care or Pharmacy Benefit Management experience; or any combination of education and experience, which would provide an equivalent background.
