The Clinical Pharmacist Specialist for Abdominal Organ Transplant is responsible for the provision of pharmaceutical care to all abdominal organ transplant (liver, kidney, and pancreas) recipients. The Clinical Pharmacist Specialist for Abdominal Organ Transplant is a designated member of the transplant team and will be assigned primary responsibility for providing comprehensive pharmaceutical care to transplant recipients in a culturally competent manner.
Works with patients and their families and members of the transplant team including physicians, nurses, clinical coordinators, social workers, financial coordinators and administrative personnel of the transplant program. Performs or oversees a team of other healthcare personnel and support staff.
Education
Bachelors Degree : Pharmacy or PharmD (Required)
Other : PGY2 Pharmacy Residency in Transplant or a relevant combination of education and experience (Required)
Experience
2 Years experience in institutional practice in organ transplant or PGY2 residency (Required)
License/Certifications
RPH-LIC - PA Pharmacist License (Required)
BCPS - Cert Pharmacotherapy Spec (Preferred)
