Overview

Location: Tinley Park but will rotate to other locations at times to cover vacations, etc.

Hours: Part-time, 24 hours/week; Monday-Friday, 3 days/week with weekend rotation every 6 weeks (when working the weekends, days offf are given during the week)

Responsibilities

  • Enters medication orders into the computer system and dispenses medications/IV solutions in an accurate and timely manner in appropriate quantities based on stability, compatibility of the drug and age specific needs of the patient.

  • Critically evaluates medication therapy and makes recommendations to improve patient medication regimens. Accurately and completely documents patient assessment data and clinical interventions.

  • Ensures appropriate and cost effective drug therapy. Areas of focus may include but are not limited to evaluating and addressing drug-drug interactions, contraindicated medications, inappropriate therapeutic regimens, adverse drug reactions, pharmacokinetics, disease state management and rational effective medication regimens according to evidence based guidelines.

  • Functions as a therapeutic consultant and serves as a resource and drug information specialist to medical, pharmacy, nursing staff and allied healthcare providers.

  • Conducts educational activities, applies principles of performance improvement, performs comprehensive clinical assessments and collaborates with the integrated healthcare team to provide quality patient care.

  • Applies the principles of continuous quality improvement consistent with job expectations. Incorporates quality improvement principles into other activities and projects (i.e. data collection, documentation). Develops criteria for drug use evaluations, medication use indicators and disease-based evaluations.

  • Provides oversight and assists with the development and implementation of evidence-based medication guidelines/protocols. Designs and implements medication monitoring guidelines and programs for clinical staff.

  • Assists the director of pharmacy with ensuring compliance with state and federal laws, validation of chemotherapy protocols and therapy plans in the EMR, and maintain competencies required for oncology practice.

  • Oversees the activities of the pharmacy technicians, coordinating workflow and may, perform the final check of medications prior to being dispensed. Ensure compliance with clean room policies and procedures in accordance to USP 797 and 800 guidelines.

  • Provides counseling and drug education to patients and families including medication therapy management. Participates in staff education via educational in-services, presentations, medical literature searches, and individual counseling.

  • Participates in inventory control including purchasing, receiving and stocking medication and pharmacy supplies.

  • Complies with governmental and other regulations related to preparation, packaging and storage of products

  • Show ability to create and utilize cost-benefit analysis and offer therapeutic alternative selections to providers when appropriate

  • Demonstrate knowledge of pharmacy regulations

  • Maintains Patient confidentiality and maintains HIPAA standards at all times.

  • Other duties as assigned by department

Qualifications

EDUCATION and/or CERTIFICATION/LICENSURE:

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited College of Pharmacy required.

  • Board Certified Oncology Pharmacist (BCOP) or Board Certified Pharmacotherapy Specialist (BCPS) preferred.

  • Current Illinois State Pharmacy License required.

EXPERIENCE:

  • 3-5 years as a pharmacist with Oncology experience

  • Hospital Pharmacy/Oncology Infusion Center experience preferred

  • Epic knowledge and experience preferred

  • Must possess ability to interpret and evaluate chemotherapy treatment protocols and non chemotherapy regimens

  • Knowledge of and competency in hazardous medication management.

  • Must possess the ability to assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements and provide pharmaceutical care based on those needs.

  • Adheres to local, state, and federal laws regarding the profession of pharmacy practice.

  • Customer service and teamwork orientation required. Mathematical skills required. Good interpersonal skills, initiative, judgment and problem solving skills.

  • Ability to organize, set priorities and plan time to accomplish workload. Ability to coordinate work between multiple sites.

  • Computer skills in pharmacy systems and word processing applications are beneficial. Demonstrates accountability and professionalism.

  • Inventory control procedures and practices

  • State laws and regulations regarding practice of pharmacy

  • Knowledge of third-party reimbursement programs and requirements

  • Knowledge of regulatory requirement related to dispensary and infusion area

  • Knowledge of USP 797 and 800 clean room standards

  • Thorough knowledge of HIPAA, OSHA and other regulatory policies and requirements
