The Advanced Clinical Pharmacist is responsible for providing all levels of pharmacy clinical services. This includes monitoring patient medication regimens and providing appropriate interventions as well as supporting all aspects of pharmacy operations. This specific role works with our Oncology teams and our clinical pharmacy to develop and oversee pertinent policies, procedures, medication use guidelines, and order sets. Serves as subject matter expert related to Sterile Product preparation and USP 797 and 800 standards and provides staff training in these areas as well as providing education specific to the Oncology patient population. Provides clinical consultation and clarification to practitioners and actively participates in daily multidisciplinary and bedside rounds. Works with the Pharmacy Director to identify and develop processes to meet or exceed all regulatory standards and participates in the continuous quality improvement of pharmacy operations.

EDUCATION:

  • B.S. or PharmD from an ACPE-accredited School of Pharmacy, a Doctor of Pharmacy from an ACPE-accredited college of pharmacy is preferred

EXPERIENCE:

  • Board certified in pharmacotherapy or other specialty board certification, AND

  • At least one of the following:

  • Minimum of 3 years of hospital clinical pharmacy experience or at least 5 years of general hospital pharmacy experience required; preferably within area of Oncology OR

  • Completion of an ASHP accredited pharmacy practice residency or fellowship in Oncology

CERTIFICATE/LICENSE:

  • Active pharmacist license in good standing with the appropriate State Board of Pharmacy
