A Clinical Pharmacist will provide patient care that optimizes the use of medication and promotes health, wellness, and disease prevention. This position will have a significant impact on cost saving and cost avoidance by working closely with patients and health care providers to provide information and guidance about the effects of prescription drugs and their dosage amounts and potential side effects, and to help plan effective drug therapy programs.The Clinical Pharmacist is also to review high cost drugs requests and validate all adverse coverage determinations before final decisions are rendered and work in conjunction with Medical Directors to render final decisions on coverage determinations. This role will serve as clinical resources for both internal and external customers in regards to the high cost drugs-prior authorization pharmacy program.

Essential Duties and Responsibilities include the following:

All processes of care/Pharmacist Referrals:

  • Regimen review, ensuring safe medication administration, monitoring for desired therapeutic outcomes, identifying and avoiding medication errors and adverse drug events, etc.

  • Provides pharmacological information by answering questions and requests of health care professionals

  • Works collaboratively with Management staff in the medication management of patients transitioning from inpatient to outpatient or next level of care

  • Counseling patients on drug therapies

  • Reduction of hospitalizations and ED visits, avoid poly-pharmacy issues, drug interactions, assistance w/medication

  • Provides clinical and educational support to staff regarding pharmacy and medication issues

Disease Management for Diabetes, COPD, CHF

  • Knowledge of chronic disease/conditions and provide treatment goals

  • To provide disease specific education to patients and improve patient participation in care

  • To educate health care workers and the community about the importance of preventive measures and medication compliance in disease state management

Post Discharge Program:

  • Reviews all medications and ensure accuracy and effective functioning and manage all communication with physicians and patients and assist to resolve all patients within required timeframe

  • Involved in the education of patients about new medications and in the education of all patients about their discharge medications

  • Discover and awareness of discrepancies, and to provide a safe and effective process for obtaining, documenting, and communicating medications across a patient's continuum of care

IDT Meetings:

  • Participate and maintain a significant role in multidisciplinary patient care rounds

Outreach Programs/Events:

  • Participate in different activities of outreach program to talk about the importance of medication management

Prior Authorization/Financial Assistance:

  • Provide support to patients and physicians when medication is recommended and prescribed but not covered by patient's health plan pharmacy benefit and lack the financial resources to pay for their medications and co pays

ACO Program

  • Reviews medication therapy to advance patient care, focusing on various approaches

  • Working together with providers/physicians to improve the quality of health care services and reduce costs

  • Advise prescribers regarding the relative effectiveness and value of their drug treatment options

  • Responsible for the ongoing management of medication therapy and outcomes measurement

The HEDIS Clinical Pharmacist will assist with quality measures related to medications and disease states. As an example, these measures include completion of medication reconciliation upon discharge from a facility, improving medication compliance/adherence for certain measures, outreaching to patients with poorly controlled diabetes to identify barriers and assist in overcoming barriers and bringing their diabetes into better control, educating patients on importance of taking statin medication for high risk patients. Will utilize clinical expertise and skills to develop the appropriate clinical criteria/guideline, policy, procedure and clinical judgment to improve pharmacy related quality measures. Will promote, assist and collaborate with our providers with clinically appropriate prescribing practices, based on evidence based medicine and national guidelines. Knowledge of pharmacy reimbursement issues and formulary management is imperative to ensure improved health outcomes to our members, while being cost effective. Reviews and analyzes pharmacy reports to track general trends. Will assist the Medical Director and serve as a clinical resource to all members of the company, providers and members. Essential Duties and Responsibilities include the following: Will be a leader, responsible for supporting and enhancing the HEDIS department, areas of involvement will include, but not limited to, Medication reconciliations for the identified populations, Medication Adherence for identified populations, health education policies and support for at-risk Diabetic members. 1. Post Discharge Program: * Reviews all medications and completes reconciliation between patient's discharge medications and current medications in accordance with HEDIS and health plan technical specifications. * Ensure accuracy and effective functioning and manage all communication with physicians and patients and assist to resolve all patients within required timeframe 2. Medication Adherence: * Core responsibilities will be related to development, implementation and monitoring of medication adherence policies in non-compliant populations * Types of interventions: side effects, cost, duplicate therapy, inappropriate dosage, medication review 3. Disease Management: * Knowledge of chronic disease/conditions and provide treatment goals, to provide disease specific education to beneficiaries and improve beneficiaries participation for Rheumatoid Arthritis, Diabetes, Hypertension, Hyperlipidemia * Implement appropriate patient education for management of HbA1C in diabetic population 4. Will work closely with Project Manager and other staff to assist in the generation and coordination of intervention for medication adherence and medication reconciliation processes. 5. Interface with Providers, pharmacists, and specialty pharmacy providers and other stakeholders as needed to achieve departmental initiatives and goals 6. Responsible for staying current with clinical literature and guidelines 7. Maintains standards set by the department and translates policies and procedures into the daily operation of the role. Responsible for acting as an information resource to patients, physicians, and other healthcare professionals regarding medication therapy and drug related information and literature. 8.
