DEPARTMENT SUMMARY: The St Charles Health System Pharmacy provides a broad range of inpatient and outpatient pharmaceutical care services to ensure safe and effective medication use.

POSITION OVERVIEW: The Clinical Pharmacist will provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care.

This position does not directly manage any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES:

Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses.

Coordinates the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians' prescriptions; ensures compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.

Mentors and trains others in training including practical experiences.

Maintains liaison relationships with medical and nursing staff; provides timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.

Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes; instructs patients in the proper use of prescribed drugs and makes Patient Care Rounds with physicians to evaluate patient progress.

Individualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.

Extracts pertinent information from patient/other sources for the patient's medical management plan.

Operates pharmacy computer system(s) and other equipment.

EDUCATION

Required: Bachelor of Science degree in Pharmacy.

Preferred: PharmD., Completion of an ASHP accredited PGY-1 residency training

LICENSURE/CERTIFICATION/REGISTRATION

Required: Current registration by the Oregon Board of Pharmacy.

Preferred: BCPS certification.

EXPERIENCE

Required: Clinical Pharmacy experience.

Preferred: N/A

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

PHYSICAL REQUIREMENTS:

Continually (75% or more): Standing and walking, carrying 1-10 pounds, operation of a motor vehicle for caregivers that are couriers.

Frequently (50%): Lifting 1-10 pounds, pushing or pulling 1-25 pounds, grasping/squeezing.

Occasionally (25%): Sitting, bending, climbing stairs, lifting and carrying 11-25 pounds,

Use of clear and audible speaking voice and ability to hear normal speech level.

Rarely (10%): Keyboard operation, stooping/kneeling/crouching, reaching overhead, pushing or pulling 25-50 pounds.

This position is for a Clinical Pharmacist role predominantly staffing in the St. Charles Bend Intensive Care Unit.

The ICU is a 24 bed unit which serves as a mixed ICU for multiple types of intensive care needs, such as Med/Surg, Trauma, and more. The St. Charles Bend Hospital is the only Level II trauma center on the east side of the Cascades, and serves a large population of patients from Bend and it's surrounding areas in Central Oregon. Pharmacists on this team participate in patient care rounding, and serve as a valuable asset to the ICU team by working on medication focused projects.

This position typically staffs a 10 hour ICU shift on a rotating 7 days on/7 days off schedule. The ICU Clinical Pharmacist is expected to review patient profiles and document pass-off to ensure continuity of care with the other Clinical Pharmacist who works the alternate 7 day period.
