Under general supervision from the Pharmacy Site Manager, the Clinical Pharmacist serves as integral team members in primary care clinics. They extend the care provided for patients with chronic illnesses, improve health and wellness, and positively impact quality metrics in patient-centered health care organizations. The main purpose of the pharmacist is to optimize health outcomes by ensuring safe and effective use of medications. In an endeavor to improve providers' efficiency and quality of care, the Clinical Pharmacist that will integrate into the patient care team in a Medicare advantage clinic.
Major Duties and Responsibilities:
Identify any barriers to patient compliance.
Design and redesign medication regimens customized to patient Characteristics and needs.
Medication focused patient education.
Assist with the resolution of medication accesses issues.
Updating custom med lists for safety and to reduce rework with wrong prescriptions.
Managing relationship with the area pharmacies to improve our communication, relationship and stay ahead of community practice changes.
Manage our medication prior authorization policy and protocols.
Educate about medication changes to include new drugs, change in indications of existing drugs and when current medications are out of favor.
Monitor and assist with management of overall prescribing trends for appropriate use of medications that may be over or underutilized
Responding to drug consultation requests.
Distribute drugs prescribed by physicians and other health practitioners.
Provide information to customers about medications and their use.
Strong focus on providing a superior level of customer service.
Log data into the computer.
Ensure compliance with all relevant laws of the applicable State Board of Pharmacy.
Administration of immunizations as allowed by state boards of pharmacy.
Any other usual and customary pharmacy duties.
Minimum Bachelor's Degree in Pharmacy
Current pharmacist license in good standing the state you are applying for
APhA Pharmacy-Based Immunization Delivery Certificate or equivalent. Genoa will provide training if necessary.
Post graduate residency experience (PGY-1) in ambulatory care.
