Demonstrates business practices and personal actions that are ethical and adhere to corporate compliance and integrity guidelines. Works closely with all pharmacy personnel to assure safe and efficient delivery of patient care. Serves in a professional capacity in-patient care areas.

Monitors patients to assure optimal therapeutic outcomes. Ensures appropriateness of all medication doses in reference to the patient's diagnosis, weight/body surface area, age, allergies, and drug interactions. Identifies and intervenes when problems involving drug therapy arise (i.e. allergies, drug interactions, therapeutic duplication, adverse drug reactions). Completes intervention documentation and submits information according to departmental guidelines.

Verifies appropriateness of drug selected. Initiates drug therapy when necessary. Suggests equally efficacious but less expensive therapy.

Evaluates need for all non-formulary medications. Assures departmental policies and procedures are being followed, medication orders are being processed in a timely manner, and the quality of products dispensed meets established guidelines. Assures the patient receives the ordered medication as to dose, route, regimen, and generic substitution.

Ensures that medication doses are filled and delivered within established departmental guidelines. Enters medication orders in the computer accurately and efficiently. Initiates and processes medication incident reports and adverse drug reaction reports as needed.

Accurately completes Controlled Substance Inventory Records when dispensing controlled substances. Completes Controlled Substance Inventory as assigned and corrects problems identified. Answers phone calls promptly and politely by stating departmental area and name.

Communicates problems, unique patient needs and other necessary information to on-coming staff for enhanced continuity of care. Acts as a liaison between the pharmacy and the medical and nursing staff for issues pertaining to pharmacy services. Supervises pharmacy technicians and support staff to promote efficient daily workflow.

Completes annual aseptic technique certification by skills validation with a qualified observer. Accurately processes outpatient prescriptions and provides oral and written counseling information to the patient and/or parent. Accurately maintains investigational drug study records and documentation.

Actively participates in the education and training of new pharmacy employees, pharmacy students, pharmacy residents, and pharmacy interns. Participates in on -going self-education activities and earns the number of hours of CE credit required by the Texas State Board of Pharmacy for license renewal. Participates in departmental data collection and reporting of Medication Use Evaluation, Quality Assurance and Quality Improvement activities.

Completes all assigned clinic and nursing station check assignments before the end of each month, including follow-up and resolution of any problems identified. Routinely checks all available means of communication including e-mail and mailbox. Performs other duties as assigned.

Education and/or Experience: Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree. Experience: One to three years of hospital pharmacy experience is preferred.

Certificates, Licenses, Registrations. Current licensure either by reciprocity or examination by the Texas State Board of Pharmacy.
