Reviews all orders for completeness, accuracy, compatibility, appropriateness and proper dosing with a review of the patient's profile such that the needs of all patient types are met (i.e. pediatric, neonatal, geriatrics). * Follows standard work for order clarification, rejects orders when appropriate and follows up with a corrected order with appropriate communication type. * Verifies physician orders, including all IV orders, utilizing standard work to ensure accuracy and proper prioritization prior to order being interfaced to nursing's bedside barcode scanning system. * Utilizes the Unverified Orders Monitor (UVOM) to prioritize order verification and designates orders appropriately for clinical follow up. * Follow standard work for processing "yellow messages" from Nursing. * Utilizes the EGH P&T approved pharmacy automatic substitution to ensure adherence to the formulary. * Calls MD to suggest a formulary substitute when non formulary item is ordered. * Fulfills duties in Pharmacy, decentralized areas, satellites, Outpatient Pharmacy, Heart City Health Center as assigned. * Follows standard work for meeting core measures as related to order verification.Dispense/Fill Cell:* Using aseptic technique, and following all USP 797 guidelines, compounds all admixtures, piggybacks, TPNs and other specialty solutions when needed according to batch times following standard work following par levels. * Direct supervision over the daily operations of the IV Room as well as the training and orientation of new staff members to aseptic techniques and IV Room procedures. * Follows and role models USP 797 guidelines in the clean room. * Prepares and checks compound sterile products with proper priority and proper labeling - "Stat" in 30 minutes, "Now" in 60 minutes, and remaining orders according to indicated time or by next route. * Properly completes chemotherapy and AIC profiles, and maintains chemotherapy, biological, syringe and epidural logs online. Completes Simplifi 797 logs. * Ensures the double check system (RPH checking RPH) is utilized at all points in the chemotherapy dispensing process and for those specific items listed as such in the compounding book. * Checks controlled substances pulled from pharmacy CSM to be stocked in Omnicell cabinets on regular routes and stock outs. * Checks all repackaging, barcoding of medications (zebra), code trays, travel boxes, and other items performed by technicians in a timely manner. * Has a general knowledge of CSM and Omnicell mechanics. * Assists in the maintenance of efficient inventory levels. * Offers constructive respectful feedback to technicians when errors are found utilizing this as a teaching opportunity. * Checks Omnicell fills for new medication uploads and Patient Specific Bin loads according to physician orders -- "Stat" orders filled in 30 minutes. "Now" orders filled in 60 minutes. All others filled in time for next route delivery.Clinical Cell:* Follows standard work for completing clinical work utilizing the patient centered list (eg: renal list, antibiotic surveillance, QT prolongation meds, digoxin, etc.) and initiating and maintaining PCPs where appropriate. * Provides clinical pharmacokinetics, TPN and anticoagulation dosing services for all appropriate patients. * Provides in-services to medical, pharmacy, and nursing staff as needed. * Documents clinical notes in Electronic Medical Record with the end result of effective and appropriate communication with the healthcare team. * Attends clinical meetings and educational in-services. * Participates in hospital committees as assigned. * Participation in clinical services demonstrates the ability to meet the pharmaceutical needs of all patient types (i.e. pediatric, geriatric, neonatal, renally compromised patients). * Performs clinical activity in the main pharmacy, decentralized areas, satellites, outpatient pharmacy, CHF clinic, anticoagulation clinic, other ambulatory clinics with pharmacist participation, CBM and Heart City Health Center as assigned. * Follows standard work to assist in meeting core measures where clinical interventions are required. * Performs patient counseling/teaching when needed. * Completes appropriate antibiotic review per antibiotic stewardship standard work. * Reviews Rehab patient profiles weekly, consulting with physicians on any recommendations). * Fulfills duties of pharmacy extern preceptor and shares clinical knowledge with pharmacy externs.Supporting Function Cell:* Assists in the development of departmental policies and procedures. * Assists in the annual review of power-plans. * Attends meetings, reads minutes, emails and stays current with all communications. * Shows initiative by initiating and completing special projects. * Assists in the department activities initiated to meet regulatory agency standards. * Provides direct supervision for technicians, maintaining continuity in workflow and efficiency. * Participates in the orientation of new staff members, orientation is performed thoroughly and cordially.Contribute to the overall effectiveness of the department * Completing other job-related duties and projects as assigned.ORGANIZATIONAL RESPONSIBILITIESAssociate complies with the following organizational requirements: * Attends and participates in department meetings and is accountable for all information shared. * Completes mandatory education, annual competencies and department specific education within established timeframes. * Completes annual employee health requirements within established timeframes. * Maintains license/certification, registration in good standing throughout fiscal year. * Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department. * Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self. * Adheres to regulatory agency requirements, survey process and compliance. * Complies with established organization and department policies. * Available to work overtime in addition to working additional or other shifts and schedules when required.Commitment to Beacon's six-point Operating System, referred to as The Beacon Way: * Leverage innovation everywhere. * Cultivate human talent. * Embrace performance improvement. * Build greatness through accountability. * Use information to improve and advance. * Communicate clearly and continuously.Education and Experience:The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a Bachelor's degree in Pharmacy or a Pharm D degree from an accredited school of pharmacy or successful completion of the foreign pharmacy equivalency exam and licensure as a registered pharmacist or eligible for licensure in the State of Indiana. The completion of hospital pharmacy residency or equivalent experience is preferred.Knowledge & Skills: * Demonstrates the interpersonal and communication skills (both verbal and written), necessary to interact effectively with physicians, hospital staff, and other health care professionals to assure optimum pharmaceutical care. * Knowledge of current pharmacy laws and regulations. * Knowledge of antibiotic stewardship principles * Knowledge of anticoagulation dosing and monitoring principles for the various classes of anticoagulants * Knowledge of parenteral nutrition dosing and monitoring.Working Conditions: * The ability to work at a brisk pace with the ability to control multi-directional problems and questions in an orderly, correct, and expedient fashion.Physical Demands: * Ability to tolerate prolonged computer screen time. * Some lifting and bending.
