Position Overview:

The Clinical Nutrition Specialist, in conjunction with other clinical staff, manages the nutritional care provided to residents in LWM. This includes screening, contributing to assessments, implementing, evaluating monitoring nutritional care plans as well as providing medical nutrition therapy according to existing policies and procedures. This person must be able to read and interpret medical records, complete documentation appropriately apply dietetics and nutrition knowledge to day-to-day scenarios. Contributes to nutritional assessments by screening residents and gathering appropriate data. Completes MDS document sections as assigned. Carries out dietary teaching plans as directed.

Essential Duties & Responsibilities:

  • Serves as a nutritional resource and role model while participating in conferences and completing day-to-day work. (Contributing to assessments, completing documentation, oversight of services, and MDS work).

  • Provides monitoring and oversight of the methods of tracking and documentation for accuracy, appropriateness, and completion with the team of caregivers.

  • Pursues ongoing training and sufficient knowledge base in dietary assessment and nutrition therapy to be capable of early awareness of dietary problems.

  • Works effectively with others and carries out prepared education plans for residents, families, and staff as needed.

  • Creates an open and supportive environment while acting as a liaison between LWM residents, families, and multi-disciplinary caregivers. Participates in and supports continuous improvement initiatives.

  • Acts as a local supervisor to food service caregivers within the long-term care area to ensure high quality, efficiency, and regulatory compliance.

  • Ongoing education and in-depth understanding of Continuous System Improvements (CSI) and supports Leaders Standard Work (LSW).

  • Acts as the liaison between the LWM staff/residents and the WH kitchen/dietary caregivers to improve processes, offer food-related events (picnics, holiday meals), relay concerns, and complaints regarding food service, etc.

  • Assist in the menu revisions to enhance the engagement of resident's choices.

  • Participate in food enhancement committees to align meal choices, nutritional needs, and meal options.

Supervisory Responsibilities:

Direct reports include the following positions:

  • LWMNA- Nursing Assistant LWM

  • FNSFSW- Food Service Worker

  • Caregivers in long-term care area assisting with meal/snack service.

Essential Skills and Experience:

Required:

  • Dietary Manager Certification or equivalent (scholarship available to receive training) within 1 year of employment.

  • Active participation in continuing education programs is expected. Ongoing education is sufficient to maintain basic credentialing.

  • Serve Safe Certification.

  • Feeding Assistance Training if available.

  • Basic computer skills. Word. Excel. Outlook. Patient information database (Cerner, MatrixCare).

  • Healthcare CPR Certification through the American Red Cross.

Preferred:

  • Geriatric Care Experience

  • Experience with completing Nutritional Assessments

  • Knowledge of the MDS (Minimum Data Set) tool for skilled nursing facilities
