Job Summary

The Clinical Nurse Liaison will possess the appropriate knowledge of Memorial Hermann programs to conduct appropriate clinical assessments of patient referrals and present assessment information to physicians; participates in marketing hospital services and continuous quality improvement with external referral sources, social workers, discharge planners, case managers, healthcare professionals, families and patients. Position is responsible for increasing the utilization of clinical services offered by Memorial Hermann.

Job Description

Minimum Qualifications

Education: Graduate from an accredited school of professional nursing, vocational nursing or licensed practical nursing

Licenses/Certifications

  • Current Texas RN license required; BSN, CRRN, CNRN or CCM preferred

  • Certificate in Basic Life Support required

  • Additional certifications will be required based on discipline and/or nursing unit requirements

Experience / Knowledge / Skills:
  • Minimum of three (3) years nursing experience in rehabilitation, ER, SICU/NICU nursing, or related field

  • Experience in similar position or Case Management strongly preferred

  • Ability to manage multi-task, prioritize work, pay close attention to details and meet deadlines

  • Because of the education and training required to obtain licensure, demonstrate principles or care of pediatric and geriatric patients within the rehabilitation environment

  • Excellent organizational and communication skills

  • Clinical knowledge to assess patients for hospital programs

  • Working knowledge of various data systems

  • Ability to effectively communicate complex ideas (both conceptual and concrete) by oral and written means

  • Ability to work independently with little or no supervision and demonstrate excellent problem solving and critical thinking skills when making decisions

Principal Accountabilities

  • Conducts clinical assessment of patient referrals and presents assessment to physicians for admission.

  • Participates in ongoing process improvement efforts related to the admissions process.

  • Serves as a resource to directors to identify processes that hinder referral source's ability to us Memorial Hermann as their facility of choice.

  • Demonstrates a complete and thorough understanding of programs and can effectively market services.

  • Participates in hospital marketing programs, events, conferences or in-services.

  • Interpersonal skills necessary in order to interact effectively with external referral sources, including social workers, discharge planners, case managers, other healthcare professionals, families and patients; Develops strong working relationships with external referral sources and internal staff including Admissions, physicians, Outpatient Clinic, Marketing staff and other departments; and ability to adhere to confidentiality rules.

  • Performs duties in a manner to promote quality patient care and customer service/satisfaction, while promoting safety, cost efficiency, and a commitment to the CQI process.

  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

  • Other duties as assigned.

SUMMARY The primary purpose of the Clinical Nurse position is to positively affect patient outcomes by providing, coordinating, and supporting evidence based direct patient care across the continuum of care. The highest quality of patient care and the appropriate use of resources is achieved in collaboration with the interdisciplinary team. The ideal candidate will have infusion and/or med-surg experience and available to work Monday-Friday with rotating weekends and holidays. Shift - 12:30pm - 11pm KEY FUNCTONS The ATC nurse has the competence and competencies regarding all aspects of administering and monitoring a variety of systemic cancer therapies and other injectable infusions to an adult population in the Ambulatory Treatment Center. Infusions administered may include (but are not limited to) standard of care and research chemotherapy, targeted therapy, and immunotherapy, blood products, IVIG, growth factors, antibiotics, electrolytes and hydration. Demonstrates excellent verbal and written communication skills. Has the ability to assess and educate patients and family/caregivers to ensure future care needs are met. Develops collaborative, professional relationships with physicians and all members of the heath care team. Specific competencies include: IV insertion and venous access device management; Accurate administration and monitoring of infusions via a variety of routes (oral, subcutaneous, intramuscular, intravenous); Performing an independent double-check for chemotherapy administration; Safe handling and disposal of chemotherapy to minimize patient and staff exposure to hazardous drugs; Continuous infusion pumps (connects and disconnects); Management of treatment related side effects and adverse reactions; Minimizing the risk for infection among patients who are actually or potentially immunocompromised; Knowledge needed to facilitate early detection of specific side effects that lead to effective self-management of the side effect when it does occur Education Required: Graduation from an accredited school of professional nursing (RN). Experience Required: None. Preferred: Two years of nursing experience; preferably chemo infusion therapy or medical surgical. Certification/Licensure Required: Current State of Texas Professional Nursing license (RN). Advanced Cardiac Life Support (ACLS) certification and Pediatric Advanced Life Support (PALS) certification if working in the following department: Nrsg Post Anesthesia Care Unit (600160).
