Position Summary:

The Clinical Nurse Liaison will serve as the initial contact with discharge planners, family or responsible party to initiate tour and admission to facility. They will also manage the discharge process and patient follow-up once the patient returns home. In addition, they will establish a strong sales presence with target audiences, including but not limited to: Acute Care providers, physicians, and others as identified within service area to promote awareness of the services for which the Community provides.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The physical demands of this position include;

  • Standing, sitting, walking, changing of position, reaching, extending hands arms, reach across midline, crouching, balancing, kneeling, stooping, bending, lifting or carrying 35 lbs push and pull 100 lbs.

Sensory Demands:

  • Auditory: to hear and understand verbal request,
  • Visually: be able to read necessary information to complete work tasks,
  • Verbally: communicate with customers and others on your team

SAFETY EQUIPMENT:

  • It is mandatory to follow safety policies and procedures including use seat belt while driving or riding in a car, when moving items as well as all personal protective equipment.

  • Mandatory reporting of all work related injuries within 12 hours to supervisor with participation in required documentation.

  • Participation in our drug testing procedures is a mandatory part of employment with this facility.

OSHA Exposure:

  • Category II, which is characterized as employees who routinely do not perform tasks that involve exposure to blood and body fluids and or tissue however in certain situations, may be placed in category I.

Position Type/Expected Hours of Work

  • Full Time position as needed under the direction of the Community Administrator

Travel

  • 90% of job tasks will be outside of facility.
Qualifications/ Education/Experience/Skills:
  • Minimal 18 years of age with high school diploma or GED

  • 3-5 years health care experience

  • Case management experience preferred

  • Valid driver's license

  • Knowledge of marketing, business skills, profession appearance and demeanor

  • Knowledge of Medicare, Medicaid, Medicaid Waiver Programs and 3rd party billing
