Responsibilities:

This position works directly with the Client Services Manager to co-manage the population served. S/He is responsible for the day to day work with patients related to in home insights and interventions needed for quality outcomes and reduction of avoidable admissions and readmissions. This position reports directly to the Director of Clinical Outcomes.

Required qualifications:

  • Current NY State RN License BSN Required

  • 1-2 years of experience in a fast-paced health services organization providing community care services.

  • Greater than 2 years of experience performing RN Assessments in the home designed for quality outcomes.

  • Flexible in scheduling to include both in office and out of office responsibilities including home visits.

  • Strong analytical, written and verbal communication skills; demonstrated ability to think critically and make decisions based on data

  • Metrics- and process-driven, passionate about numbers as well as people

  • Motivated self-starter and creative problem-solver who is comfortable working in a fast-paced, dynamic environment

  • A genuine, compassionate desire to serve others and help those in need
