Responsible for managing a staff of


30 including Case Managers, Social Workers and Psychiatric Mental Health NPs as well as 2 Supervisors one for the Main Campus#and one for the Saints Campus of Lowell General Hospital.# Will work closely with the Director of Case Management#and take responsibility for the department#systems functions, staff performance, and patient care outcomes.# Will lead through change both planned and unplanned, build consensus when appropriate, motivate and encourage others. Responsible to maintain an inclusive workplace; maximizes contributions of all employees, and develop strengths in all team members. Responsible for hiring, counseling and coaching staff, providing disciplinary actions, positively interacting with medical staff and support services during times of stress and constant change. Experienced in managing Performance Improvement Plans, # Addresses any concerns/issues or patient complaints with the patient, family, and staff involved as needed. Oversight of case management clinical leaders for complex discharge planning Assists the Director with PI projects, data collection, presentations for quarterly meetings and/or consent agendas Assists the Director with alignment, transformation and collaboration with providers and community partners This is a full time, exempt position: 8am- 430pn with on call weekly rotation every 4th week/weekend The ideal candidate will have: BSN required, MSN preferred. # BLS#and ACLS required and case management certification Prior proven experience and expertise in case management#(minimum 5 years clinical work) in a hospital setting Prior proven experience and expertise in management, either in first line or in shift supervisory work. Excellent verbal and written skills for high level communication within the hospital and at interdisciplinary and administrative meetings. Experience developing written proposals, preparing budgets, writing evaluations and developing standards and Performance Improvement (PI) reports.   • Ensures implementation of the nursing process and established nursing standards

  • Observes and monitors compliance of nursing staff in the daily practice of care delivery

  • Demonstrates standards compliance through staff documentation in the medical record

  • Ensures staff compliance with annual re-certification program

  • Ensures staff compliance with competency education and age specific programs

Staffing:

  • Maintains appropriate levels of staff and assignments and employees satisfaction according to organization standards.

  • Maintains personnel congruent to the approved staffing mix and budgeted positions:

  • Assists department director in development of area budget

  • Maintains position control for assigned areas

  • Recruits, interviews, recommends and hires qualified applicants for employment

  • Conducts staff performance appraisals within designated time frames

  • Provides counseling and corrective action as appropriate to staff following HR protocol

  • Collaborates on personnel action issues with Department Director and HR

  • Evaluates unit or department turnover rate and reasons for termination and implements appropriate strategies as indicated

Education and training:

  • Creates and fosters a learning environment that facilities the professional growth and development of staff

  • Collaborates with the clinical educator to assess learning needs and ensures the provision of appropriate education programs

  • Identifies and communicates to clinical educator and director educational needs based on evaluation of personnel's clinical practice

  • Collaborates with educator to evaluate the orientation program of new staff

  • Collaborates with educator to determine and monitor

  • competency based evaluation programs

  • Fosters development of staff by encouraging participation in policy making, participating in the team process, and resolving unit concerns

  • Anticipates potential problems of staff or intradepartmental staff and intervenes to offset diverse outcomes utilizing the problem solving process, team building techniques and demonstrating a proactive attitude

  • Assists with providing learning experience for staff

  • Provides opportunities for individual discussion of staff development needs

  • Models interpersonal behaviors to build teamwork and maintain an environment of trust among diverse staff, with administration, and other units and physicians

Leadership Capacity:

  • Demonstrates effective leadership skills of delegation, organization and coordination in managing unit

  • Conducts regular supervisory rounds as applicable to include the following monitoring of staff documentation, patient and customer satisfaction, patient education, nursing care and unit services provided, implementation and use of protocols, and overall work environment

  • Program Development:

  • As a member of management team, participates in identifying needs at the unit and organizational levels and assists with strategic planning, organizing, executing, controlling and evaluating systems, processes and practice, patient care and information processing

  • Actively participates in at least one community based program or organization yearly

Budget Planning, Monitoring & Stewardship:

  • In collaboration with department director or Executive, assists in the budget planning and execution for unit(s) in support of organizational budget

  • Identifies unit resources and needs; administers and evaluates the area budget, with emphasis on monitoring expenditures in supplies, equipment, staffing and cost control measures

  • Reviews responsibility reports, productivity cost and data, contract workers costs and upcoming needs for capital expenditures

  • Provides variance reports as indicated

  • Actively identifies opportunities for more efficient use of resources

  • Under unique situations may be responsible for budget planning, development, and proposals

Standards & Policies, The Joint Commission (JC):

  • Responsible for compliance with all standards and policies

  • Reviews, revises and participates in the development of structure, process and outcome such as: Standards at the department and hospital level and uses these standards as a basis for all program and professional development and outcome accomplishment accordingly

  • Demonstrates awareness of Hospital policy, assures compliance with and uses these as a basis for decision making for both clinical and management issues

  • Maintains all necessary unit manuals to include: Performance Improvement, MSDS, Infection Control, Generic and Unit

  • Structure Standards, Personnel, Safety Management, Risk Management, Human Resource, and Administrative

  • Actively participates in JC accreditation process for the department or unit which includes 'mock' survey activities, intervention as appropriate and assists in the follow up and corrections based on recommendations

  • Actively participates on at least one Functional Team

Staff Development and Performance Improvement:

  • Develops and retains extraordinary employees through coaching and performance feedback resulting in mutual trust and respect:

  • Coordinates all aspects of Performance Management including:

  • setting expectations, continuing education, credentialing, performance appraisals, auditing, concurrent monitoring, utilization review, risk management and active problem identification

  • Demonstrates ability to collect, track and trend data in order to identify opportunities for performance improvement

  • Performs required performance appraisal on time for unit staff based on hospital job descriptions and unit based performance standards

  • Assists in identifying, planning and implementing educational programs based on staff needs. Evaluates effectiveness of educational programs based on performance improvement

  • Demonstrates the ability to plan contents and effectively delivers educational information of a clinical nature (as applicable) and regularly participates in direct

  • staff learning activities

Working Relations and Team Communications:

  • Develops and maintains effective communication mechanisms with staff, physicians, patients and families; between unit and other departments; and between area staff and members of the Management & health care teams

  • Attends and participates on committees, task forces, and meetings for coordination/facilitation of patient care activities or

  • unit services at the department level as required

  • Holds staff meetings, as requested, prepares an agenda including required elements

  • Attends regular meetings

  • Promotes cooperation and collaboration with other departments when working to accomplish goals

  • Develops team spirit within unit and assists in developing relationships between teams

  • Assists in implements strategies for participative management involving staff in general decision making for hospital/unit operations/clinical practice/patient care via unit based committees as necessary, staff meetings and participation in standards development, PI, CE and competencies

  • As applicable, advises and consults within the unit regarding the identification of software and information infrastructure problem areas, determines procedural changes or application program modifications, and assists with problem resolution.

  • In the absence of the area director, assumes full responsibility for area productivity and performance.

Safety and Legal:

  • Ensures that patient care, nursing practice or services provided in a unit meet safety, risk management standards and legal requirements of licensing

  • Demonstrates awareness of adherence to safety and legal requirements established at the hospital and unit levels with emphasis on minimizing risk to staff and reducing Rate Based Indicator (RBI) events in patients

  • Ensures staff compliance of meeting licensure requirements, OSHA, LOA, MLOA and injury guidelines

  • Monitors staff compliance of mandatory monthly safety in-services as required

  • Ensures quality, professional nursing care is provided in pre and post care areas

  • Identifies the number and level of personnel needed to provide patient care, as defined by federal and state regulations, and adjusts staffing and assignments appropriately

  • Coordinates the activities of staff and patients

  • Evaluates the quality of nursing care delivered by assigned staff, identifies problems and/or areas requiring corrective action, facilitating the performance improvement process and maintaining regulatory compliance

  • Guides and directs staff regarding patient care performance through clinical expertise or mentoring

  • Maintains standards for patient care outcomes

  • Maintains budgeted staffing plan adjusted to volume and acuity
