Experience: Two years of community health experience in a Medicare certified agency. Demonstrated clinical competency and current knowledge of the principles and practices of home care. Healthcare management experience preferred. Demonstrated familiarity with DPH and Joint Commission regulatory standards preferred. Demonstrated coaching/team building skills required. Education and Training: Associate#s Degree in Nursing or DPT, PT, OT SLP from an accredited program required. BSN in nursing preferred. License, Certification

Registration:

RN/Rehab- Licensed as an RN, OT, PT, ST in MA. OASIS#- OASIS#Certification within first two years in the position. Other Requirements: Ability to exercise independent judgment. Demonstrated interpersonal skills and problem-solving ability. Ability to supervise, coach and mentor and motivate others. Ability to maintain confidentiality. Ability to be flexible regarding working hours. Knowledge of community resources. Must possess a valid, current MA#driver#s license and be able to drive in inclement weather. Exemplifies Customer Service Standards. Excellent interpersonal skills and ability to handle conflict required. Demonstrated flexibility both in thought process and in time availability.
