Responsibilities: Ensure effective intake and referral processes Complete admissions process for referred patients Provide accurate information about hospice services Act as a patient advocate in evaluating and developing care plans Engage with various healthcare facilities and provide education on the benefits of hospice Requirements: RN license in the state of employment Minimum of 2 years healthcare experience, preferably in hospice or home health Known and respected within the community Company Information:
