Job Summary:

This position acts as a liaison between internal and external referral sources, and internal operations. The Clinical Liaison is responsible for establishing relationships with physicians, payers, and other referral sources. The Clinical Liaison will ensure appropriate patient placement through clinical assessments and screening in conjunction with the Hospital's Medical Director.


  • College degree and license in healthcare-related field (RN), Therapist

  • Clinical experience required with demonstrated skills in clinical assessment and marketing.

  • Minimum of 2 years experience in acute care or rehab.

  • Requires a professional level of knowledge in physician relations and interfacing with insurance companies.

  • Extensive knowledge of local provider and medical communities.

  • Ability to be available for occasional evenings for marketing events, weekend coverage, and routine day travel.

  • Good communication skills.

  • Ability to work harmoniously with other personnel.

  • Ability to deal tactfully with, patients, hospital staff and the general public.

  • Must be able to follow written and oral instructions.

  • Must maintain valid Ohio driver's license.

The Clinical Coordinator is responsible for performing, coordinating, and directing nursing care and services in a chronic-care facility for the end stage renal disease patient in accordance with DaVita Healthcare Policies, Procedures and Guidelines, OSHA, CMS, federal, state, and local regulations. This hourly position reports to the Facility Administrator and typically works 40-50 hours/week. Clinical Coordinator Requirements * Nursing Degree from accredited school of nursing required * Proficient in all RN I and RN II duties and has more than one year of nursing experience * Current RN license in state of practice * Current CPR certification required * Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system * Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in all computer systems of DaVita within 90 days of hire required Clinical Coordinator Responsibilities * Supervises and monitors direct patient care administered by PCTs, LPN/LVNs and RNs by directing and assigning the work of PCTs, LPN/LVNs, and RNs, using independent judgment to assign teammates to patients as deemed appropriate by the Clinical Coordinator * Ensuring appropriate documentation of all delivered care * Ensuring that care is provided in compliance with DaVita's policies, including Operational and Teammate Policies * Ensuring that care is completed * Responsible for the performance and duties of clinical teammates who are being directed and assigned * Assist FA with evaluation and counseling of clinical teammates as well as annual evaluation of clinical teammates' performance * Creates and changes teammate schedules as necessary, using independent judgment to determine which teammates will treat which patients, as necessary * Performs direct patient care including but not limited to taking and monitoring vital signs, initiation and termination of dialysis, documentation, physical assessment of patients and patient treatments, taking and transcription of physician's orders, drawing and documentation of ordered laboratory work * Notes signs of trauma, patients' general physical and mental condition, reviews dialysis orders, documents observations in patient medical records * Analyzes patient assessments, laboratory data and, when concerns are noted, brings issues to the attention of the physician * Communicates all emergencies, patient life-threatening complications and related incidents to supervisor, Medical Director, Regional Operations Director, Facility or Group Facility Administrator * Assesses patient and family learning readiness and provides appropriate information on renal disease, prescribed dialysis treatment, and medical regimen and its impact on the patient's state of health and well-being * Provides dialysis therapy, including but not limited to formulating dialysis plan per physician orders, weight and rate of ultrafiltration, verifying safety measures are in place and complied with, administering medications or other appropriate treatments, discontinuing dialysis according to established procedure, documenting all activity appropriately * Instructs patients on pre- and post-dialysis care * Administers patient quality care in a considerate, respectful manner * Participates in the development and implementation of the patient's short and long term plan of care and monthly progress note * Proficient in all RN I and RN II duties; is a resource for new RNs * Provides unsupervised patient care * Demonstrates the ability to transfer skill set to other clinics and works at other clinics when required * Meets qualification for Charge Nurse; CQI program and outcome management * Facility Health Meeting (FHM) leader and driver for improvement * Is a Kidney Smart (Patient Education) Program Captain or manager * Is a resource for colleagues with less experience * Ensures the transcription of all physician's orders * Ensures the documentation of all laboratory work orders A facility may be selected to participate in research study(ies) under the guidance of DaVita Clinical Research.
