To educate and promote superior quality, competitive value and the outstanding services of Health First Hospice and Home Care Services to the Brevard County community and its medical providers. Position will oversee all educational activities for HF Hospice Services and Home Care Services, including collection and analysis of education/clinical-related research data; development of specific educational programs, and promoting goals through education. Communicates, tracks, and reports results of educational activities to Hospice and Home Care Directors.

  • Serve as a resource for community on Hospice and Home Care services representing them through participation in support groups, community educational offerings and related one on one meetings.

  • Establishes him/herself as an integrated partner with case management team at designated Health First hospitals.

  • Regularly attends weekly case conferences at designated hospitals to identify patients meeting criteria for Home Health and/or Hospice Services. Educates hospital case management staff on importance of early identification of Hospice and Home Care patients and encourages urgency to place patients in appropriate services as soon as medically appropriate.

  • Serves as ongoing resource to case management staff regarding appropriate admission criteria, regulatory considerations/changes.

  • Serves as an ambassador to case management staff regarding benefits and quality of Health First Hospice and Home Care.

  • Appropriately escalates potential areas of conflict between internal stakeholders in order to encourage expedient solutions in the mutual best interest of Health First.

  • Responsible for assisting with projects and presentations within the community to promote community awareness and wellness.

  • Represent Health First in professional, ethical matter at all times.

  • Promote/Educate Hospice and Home Care Services on approved service line topics to hospitals, case managers/discharge planners, physicians and various organizations on Health First Hospice and Home Care Services.

  • Responsible for serving as a liaison between the organization and physician, etc. to ensure continuity of patient care

  • Attend appropriate Health First, Physician, Case Management and Directors Meetings to problem solve any barriers to business

  • Obtain and organize community related information and provide input on customer opportunities

  • Build and maintain client relationships

  • Maintain a cohesive team with referral sources by responding to calls quickly and positively

  • Performs formal presentations on Hospice and Home Services to physicians, medical groups, case managers and community organizations.

  • Demonstrates computer proficiency in applications and presentations to others.

  • Knowledgeable of HCFA and AHCA guidelines and able to educate physicians, medical personnel and community organizations.

  • Maintain knowledge of clinical applications (such as Centricity, Curaspan and Horizon)

  • Monitor and report on competitive environment regularly

  • Monitor and report of referral source expectations and potential referral source obstacles

  • Participates in defining tactics and business strategy

  • Achieve growth with Hospice and Home Care Services

  • Ensure all PHI information is protected per HIPPA guidelines and department processes

  • Coordinate case management with appropriate representatives to ensure eligibility for services to facilitate payment of claims

  • Preferred minimum of 3-4 years Hospice/Home Health experience

  • Excellent presentation skills

  • Preferred knowledge of Medicare/JCAHO Regulations

  • Excellent written and verbal communication skills

  • Excellent problem solving skills

  • One year experience in working with computer applications.

  • Demonstrate the ability to relate well with all customers (patients, family members, physicians, Health First Associates, Community Leaders).

  • Valid driver's license, automobile insurance and reliable transportation

VIEW JOBS 10/16/2020 12:00:00 AM 2021-01-14T00:00 POSITION SUMMARY The clinical applications educator works directly with clinical application non-provider end-users, business owners, business systems analysts, and clinical informatics to implement education and training strategies for new systems and modifications to existing systems as outlined in the Clinical Applications Education (CAE) catalog under the direction of the manager. This position also assists with developing the end-user workflow, system design, & basic functionality, troubleshooting and application optimization. This role as functions as an advocate for the application end-users for applications outlined within the CAE catalog. This role also supports strategic projects as a crucial member of the project team by providing onsite support for the end-users for applications supported in the CAE catalog. PRIMARY ACCOUNTABILITIES * Coordinates, develops, and delivers clinical application education training materials through various modalities. * Participates in developing & delivering clinical application education to end-users. * Successfully builds customer rapport and serves as the liaison between IT and customer regarding clinical application functionality. * Serves as the subject matter expert with clinical application functionality. * Maintains accurate documentation of performed functions through information technology service management and administrative tools. * Recommends innovative ideas, processes, and/or procedures for enhancements, additions, and modifications to improve and optimize end-user application workflow. * Presents advanced knowledge of expertise in clinical workflow functionality documentation. * Provides first tier customer-facing application functionality support through various modalities. * Manages clinical application training rooms, training environments, and hardware. * Participates and represents the department in organization-wide committees and meetings. * Participates in sharing trends and advances in application training, informatics, and the electronic health records. * Supports end-users during downtimes and major projects. * Performs other duties as assigned.
