As a Clinical Liaison for Integritus Healthcare, supporting our affiliates on the North Shore of Boston, you will be an integral component of how we provide essential and needed services to those individuals needing short-term rehabilitation or long-term skilled care. You will be our point person for coordinating and delivering on critical aspects of how our referred patients are reviewed and approved for placement in our affiliates.

Your excellent computer, communication, and organizational skills will be critical to obtaining authorizations from insurance companies, completing data reviews and document uploads from hospitals, and ensuring complete screenings and PASRRs before the resident transfers. You will also be trained and prepared to support fellow liaison team members to screen patients clinically and correspond with hospitals when needed.

The referral coordinator is primarily responsible for referral coordination and communication through acute care organization referral and referral management systems. Clinical Liaison's responsibilities of screening and corresponding with critical care providers occur on an as-needed basis, as determined by the department and organizational focuses.

Responsibilities:

  • Completes essential referral input per the specific RACI tool outline for new referrals from acute care settings or direct facility referrals faxed into RMS.

  • Verifies payor source for all accepted referrals scheduled for admission providing referral manager with any payor-related issues or concerns.

  • Conducts insurance authorizations for accepted commercial insurance referrals scheduled for admission, including negotiating rates where applicable.

  • Aids referral manager for clinical screens, PASRR completion, or bed-hold updates as requested.

  • Supports and executes client/clinical liaison duties on an as-needed basis as determined by department and organizational leadership.

  • Functions as system administrator for the various electronic referral systems within the market assigned.

  • Monitors electronic referral systems for updated documents and attaches clinical documents into RMS patient records, as necessary.

  • Works collaboratively and cohesively with the various teams, including facility-based and Liaison teams, to understand 5 Star and Service mix goals and assist with ideas and plans supporting the common objectives.

  • Understands and works in support of the organization's mission, vision, and values, always conveying a positive image of Integritus Healthcare and its facilities to all audiences

Job Requirements

Qualifications:

  • Successful experience of 1-2 years in a healthcare setting preferably LTC-related. Administrative, nursing, social work, or liaison experience preferred.

  • Associates degree in a health related or LPN, LCSW, or RN license required

  • Fully vaccinated and boosted

  • Valid Driver's License

  • Basic understanding of commercial insurances, Medicare, and Medicaid reimbursement. Advanced computer experience with Microsoft, Excel and clinical or sales software.

This is to be accomplished by the provision of an environment consistent with participants' needs in daily living and those skills necessary for community life.</p><p>Direct Care Worker I POSITIONS AVAILABLE IN THE FOLLOWING</p><p>Berkshire Area CITIES &amp; TOWNS:</p><p>Lee | Pittsfield | Stockbridge</p><p>Duties and Responsibilities (these duties are a general summary and not all inclusive):</p><ul><li><p>Provide support to individuals by assisting in the development and implementation of habilitative, behavioral and recreational programs in accordance with the Individual Service Plan (ISP) objectives</p></li><li><p>Coordinate with direct supervisor(s) to ensure successful implementation of ISP's, use of appropriate methods, coordination of services and crisis intervention</p></li><li><p>Assist and facilitate activities of daily living, such as household maintenance, self-care and hygiene, food shopping and preparation, budgeting, leisure activities and socialization skill development</p></li><li><p>Attend individuals' review meetings and accompany them to various appointments; Transport individuals to work, recreational activities and other engagements as needed</p></li><li><p>Maintain individuals' confidentiality and records; preserve individuals' human rights by complying with applicable laws and regulations</p></li></ul><p>Preferred Qualifications:</p><ul><li><p>Demonstrated ability to develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with individuals with intellectual and developmental disabilities;</p></li><li><p>Capacity to assist/lift individuals with physical disabilities and/or limited mobility;</p></li><li><p>Aptitude to transcribe, read and comprehend the English language</p></li></ul><p>Agency Requirements:</p><ul><li><p>Possess current and valid Class D Motor Vehicle Operator's License</p></li><li><p>Applicants must pass the Medication Administration Program (MAP) certification</p></li><li><p>Upon successful completion of a training program, incumbents must obtain and maintain Cardiopulmonary Resuscitation (CPR) and First Aid (FA) certification</p></li></ul><p>Please Note:</p><p>DDS fills this entry-level position on an ongoing basis as staff turnover and new program development occurs. The Department has full and part-time positions available on various shifts. Many of the positions require working every weekend with two days off in the middle of the week. Applicants must be flexible in work schedule. This is a 24-hour/7 days a week staff position. The Department requires employees in this position to be able to perform mandatory overtime when circumstances dictate the need.</p><p>This position is designated as an &quot;EMERGENCY&quot; position in the Department of Developmental Services Emergency Plan.
