Job Description:

The Clinical Intake Coordinator facilitates the hospice and home health referral process by receiving referrals from multiple referral sources within and without the Saint. Luke's Health System, performs initial evaluation of referral for home health appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines need for additional information from referral sources, communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals.

Facilitates and promotes the Tele-Health services for the agency. This position may require travel.

Basic Life Support - American Heart Association or Red Cross, Licensed Practical Nurse (KS) - National Council of State Boards of Nursing (NURSYS), Licensed Practical Nurse (MO) - National Council of State Boards of Nursing (NURSYS)
