Job Description: #The Clinical Improvement Liaison manages clinical performance improvement and quality activities for LVHPHO/Valley Preferred. This position works directly with Primary Care providers, Specialists and APCs and their practice/office staff to support clinical improvement activities that will allow them to achieve and maximize clinical, quality, and financial performance goals for the membership population.
This position interacts with physicians, administrative leaders, and other clinical staff and provides support and guidance to clinical improvement initiatives. The Clinical Improvement Liaison is responsible for assisting the organization in adopting best practices to include alignment with payer contracts and the Centers for Medicare and Medicaid. This individual has subject matter expertise in continuous quality improvement and can work with all health care providers and their practice/office staff to identify and design new processes and practice workflows that will enable the transformation of care delivery to better support practices in achieving goals.
This position is responsible for developing and maintaining a cooperative working relationship with LVPHO/Valley Preferred Valley Preferred constituencies, including managing the data and reporting needs related to relevant programs for all payers and providers. This position optimizes clinical collaboration with payers and coordinates with all relevant internal staff as well as external participating providers. Individual will provide vision and expertise necessary for the stability, growth and success of the relationship with all contracted payers.
#Bachelor#s degree in health related field required. # Broad understanding of the health care environment and clinical performance and its essential tie to all health care providers. Excellent oral and written communication skills, including the ability to translate detailed patient-level data into meaningful management reports for both clinical and non-clinical audiences.
Must have strong commitment to service and achieving optimum status with payers. Ability to manage multiple priorities and interact with multiple people at a variety of levels.
Master#s degree.# # Strong analytical skills with the ability to define data needs for both clinical and non-clinical support teams (e.g., for Administrator and/or medical directors and for information technology staff). # Licensure Required: RN
