This is a remote position!
Clinical Home Care Coordinator is primarily responsible for reviewing all homecare orders, certifications, and recertifications for accuracy, contacting patients to gauge level of satisfaction with designated homecare agency, communicate with PCP regarding orders, obtain signatures from PCP, document and submit billing as appropriate, and work with other care coordinators. S/he must exhibit strong interpersonal and communication skills as well as the ability to utilize both internal and external resources to accomplish the goals and objectives.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Review all homecare certifications, recertifications, and orders received from homecare agencies and review for accuracy to meet patients needs
Communicate with patients, PCP and specialists
Obtain signatures from PCP on documentation
Complete all necessary documentation for billing requirements
Work with homecare agencies to coordinate care as needed
Meet with homecare agencies within the designated area to build relationships as needed

Education and/or Experience: RN or LPN
Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints.
To perform this job successfully, an individual should have knowledge of Athena Database software; Microsoft Spreadsheet software and Microsoft Word Processing software; including Excel.
