JOB DESCRIPTION: SUMMARY:# Under the supervision of the Manager of Coding and Clinical Validation, the Clinical Documentation Improvement Educator is responsible for providing education and training to Clinical Validation staff, physicians and the health care team on effective, compliant clinical documentation.# Facilitates consistent, accurate and optimal documentation by providing both formal and informal educational programs.Assesses documentation gaps and formulates appropriate CDI educational tools to include, but not limited to, PowerPoint presentations, newsletters and documentation tips sheets. Applies teaching/learning principles in establishing an overall educational program related to effective clinical documentation for, and in collaboration with, Clinical Validation staff, physicians and the health care team. Acts as a resource to Clinical Validation staff and clinical providers on optimal documentation.# # Education Bachelor#s Degree, preferably in Nursing or Health Information Management# Experience Five years# experience as a Clinical Validation Specialist and/or educator Skills Must demonstrate understanding of the APR-DRG system, impact on case mix index as well as ICD-10 coding systems and how guidelines relate to clinical documentation improvement. Demonstrates competency in teaching/learning process. Demonstrates extensive clinical knowledge and understanding of anatomy, physiology and pathophysiology.# Must have a thorough understanding of clinical documentation principles, guidelines and regulations.# Must be able to communicate effectively with peers, physicians, nursing and other staff. Demonstrated interpersonal skills that promote teamwork. Must demonstrate abilities to train and deliver education programs. Must possess strong verbal and written communication skills, and be able to produce training presentations and materials. Licensures, Certifications RN licensure CCS/CIC required; CCDS and/or CDIP strongly preferred but required to obtain within a year of hire Physical Requirements Ability to sit, stand, walk, concentrate and pay close attention to detail. General Summary Under general supervision serves as liaison to attending, resident physician and Nurse Practioner staff for the Clinical Documentation Initiative. Facilitates Physician/Nurse Practioner completion of retrospective documentation queries. Provides informal education to physician staff regarding documentation and coding requirements. Supports coding/documentation guideline initiative and obtains physician feedback. Compiles and analyzes information to assess the quality of physician medical record documentation and its subsequent impact on severity leveling and reimbursement. Uses existing databases to develop and produce standard and ad hoc reports that support clinical documentation improvement. II. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified. 1. Attends Clinical Development Initiative oversight meeting or meetings with physician service groups as requested by Manager. 2. Facilitate physician completion of queries which were not addressed during the hospital stay or are being made retrospectively by Health Information Management (HIM) or outside agencies. 3. Assesses physician concurrent documentation query response through closed chart review / Online Transcription Information System (OTIS) and enters findings into relational database. 4. Performs appropriate analyses of query and documentation patterns/trends. Identifies actual or potential issues and reports findings to CDI manager. 5. Participates in Physician/Nurse Practioner education regarding documentation and changes in coding / CDI requirements. 6. Meets established productivity targets for query compliance, retrospective record completion/quarter close, and Physician/Nurse Practioner query answer completion. 7. Participates in documentation/coding guideline development and obtains Physician / Nurse Practioner feedback regarding guidelines as required. 8. Conducts documentation/coding reviews and analysis as requested for specific areas/projects. 9. Monitors and documents Online Transcription Information System (OTIS) query responses appropriately in CDI database. What You Need to Be Successful: III. Education and Experience 1. Bachelor's degree in Business Administration, Health Services Research, or a related field is required, or equivalent experience in coding, chart review or medical record keeping is required. 2. Two years experience working in coding, chart review, and/or medical record keeping in a healthcare setting is strongly preferred. IV. Knowledge, Skills and Abilities 1. Knowledge of medical terminology, chart review or health related experience is required. 2. Knowledge of AHIMA (American Health Information Management Association) practice briefs on query compliance and CDI best practices. 2. Proficient knowledge of spreadsheets, word processing and relational database (MIDAS), OTIS, HPF (Horizen Patient Folder), and Powerchart. 3. Excellent analytical skills, with the ability to prioritize work and work independently. 4. Demonstrated ability to work collaboratively with other team members. 5. Highly effective verbal and written skills are required to work successfully with the physicians and various levels of administrative and management personnel.
