These clinical RD positions are responsible for providing outpatient nutrition care in the Katy and Sugar Land Houston Area Locations (HAL). This Clinical Dietitian works in coordination with the Katy or Sugar Land HAL Senior RD to communicate and collaborate with physicians and health care team members at the HAL, the Administrative Team for the Department of Clinical Nutrition, and other HAL and main campus Dietitians to provide care during all stages of treatment and attain departmental goals.
This position is responsible for providing direct nutrition care to patients with complex medical conditions. This includes communicating recommendations for nutrition intervention in the electronic medical record and during HAL interdisciplinary rounds; as well as providing nutrition education to patients and health care professionals.
The HAL dietitians work with each other to cover the services of the HAL dietitians which may require traveling to another HAL location. Dietitians also provide education on nutrition and cancer for the community as part of support groups and outreach activities of the Houston Area Locations.
Provides quality nutrition care in relation to the patient's diagnosis, treatment, age group, nutrition status, and other needs in both an acute care environment, and/or within the clinics as appropriate.
Provides a comprehensive assessment of nutritional status by synthesizing and analyzing information obtained through personal interviews, physical assessment, review of the medical record, consulting with other members of the care team and via reports (i.e. laboratory and other diagnostics) retrieved from the computer.
Determines patient nutrient needs and develops a comprehensive plan of care by using department-established guidelines.
Provides coverage to other HAL's to assure continuity of patient care in the absence of the center Dietitian.
Collaborates with the interdisciplinary team individually and in meetings in recommending, implementing, and monitoring of nutrition interventions.
Communicates care of patients to team in HAL interdisciplinary conferences and daily management of patients.
Documents care in the medical record using appropriate forms and/or formats within established guidelines.
Provides initial and ongoing or follow-up care according to established time frames and reassessment criteria.
Identifies and provides for patient and family education needs, assesses barriers to learning, provides appropriate instructions utilizing appropriate modalities, assesses patient understanding and documents interventions on the Interdisciplinary Plan of Care and Teaching Record.
Provides professional and staff education to ensure the safe management and care of patients and families by clearly presenting formal and in formalin-service classes in HAL's and by providing clearly understood written and/or printed information. May provide written and verbal information at professional conferences and seminars.
Required: Bachelor's degree in nutrition or dietetics or a related field with completion of Academy ofNutrition and Dietetics approved Dietetic Internship or its equivalent.
Preferred: Master's Degree in nutrition, a related field, or business.
Preferred: At least 1 year experience in an acute care setting in a tertiary, acute care teaching/research facility.
Required: Current Registered Dietitian (RD), Commission on Dietetic Registration, and Current Licensed Dietitian (LD), Texas State Board of Examiners of Dietitians
Preferred: Certification in Oncology or similar advanced practice certification relevant to position (i.e., nutrition support, geriatrics, pediatric, renal, diabetes education).
