As a Clinical Coordinator, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll assist members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher, and more. The result? Fewer hospitalizations, ER visits, and costly service gaps, and a less stressed, more effective health care system for us all.

Primary Responsibilities:

  • Receive, evaluate, accept, and execute the initial patient intake process to assure that patient verification, assessment, teaching, and ongoing scheduling of nursing services occur in timely manner

  • Assist members with the transition from a care facility back to their home

  • Provide a complete continuum of quality care through close communication with members via phone interaction

  • Support members with condition education, medication reviews, and connections to resources

  • Assure timely communication with other company personnel and outside providers to assure the quality and continuity of patient care

  • Up to 20% direct patient care

Required Qualifications:

  • Current, unrestricted RN license in the state of NY or the ability to obtain prior to employment

  • 2 years of clinical experience in a hospital, acute care, home health / hospice, direct care, or case management

  • 1 years of infusion experience

  • Computer / Typing proficiency to enter / retrieve data in electronic clinical records; Experience with email, internet research, use of online calendars and other software applications

  • Willing and able to meet attendance, after-hours on-call, and other reliability requirements of the position

Preferred Qualifications:

  • Bachelor of Science in Nursing

  • Home infusion experience

  • Home health experience

  • Case management experience

  • Pediatric experience

  • Certified Registered Nurse Infusion (CRNI)

  • Knowledge of INS (Infusion Nursing Standards) and nursing process including assessment, problem identification of nursing diagnosis, planning and communicating, implementation, and evaluation

  • Knowledge of NY State LHCSA regulations

Primary Activities and Responsibilities * Ensure compliance with all train orders, signals, railroad and safety rules and Federal Railroad Administration (FRA) regulations * Be able to safely perform all job responsibilities * Inspect all equipment on cars prior to departures * Receive and transmit information by radio and telephone * Read and understand bulletins, work orders, and switch lists * Assist with the coupling and uncoupling of cars, operate switches, and make minor repairs to railcars, including replacing heavy couplings or air brake hoses * Work with customers to ensure accurate movement and placement of cars; delivers excellent customer service by identifying customer needs and meeting or exceeding expectations * Maintain awareness of surrounding area and suspicious activity to ensure rail safety * Operate locomotive equipment through the use of remote control device * Quickly and repeatedly make precise adjustments in moving the controls of a machine or vehicle to exact positions * Comprehend color and/or hand signals and systems * Review instructions from dispatchers and other transportation employees, and discuss with locomotive engineer * Miscellaneous activities and responsibilities as assigned by manager * Pay Rate $43,000 average first year earnings Training rate while in Atlanta $728.24/weekly (north) OJT Training rate $874.14/weekly (north) Training You will attend three weeks of training at the Railroad Education & Development Institute (REDI) in Atlanta, GA.
