As a Clinical Coordinator, RN for our Intensive Care Unit you will be a vital member of a dynamic team responsible for responsible for the complex, multi-disciplinary coordination of specialty patient population(s), developing, evaluating, and revising care pathways based on assessments conducted through engagement with the patients and families in the ICU. Summary of Responsibilities:
Accountability for the coordination of nursing care, including direct patient care, patient/family education and transitions of care
Support professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family
Educates and serves as an expert resource for other personnel
Contribute to the strategic planning process and attainment of goals of the organization
Function within the Day Kimball model, which includes accountability for planning, implementing, evaluating, and communicating all phases of nursing care for assigned patients
Provide leadership through activities such as preceptor role, informal leadership roles, and quality improvement efforts
Delegate patient care according to skill level, experience, patient acuity, fiscal accountability, and adequacy of resources
Mentor and coaching to nursing staff; and may participate on committees or projects
Knowledge, Skills, and Abilities:
Ability to recognize unsafe and life-threatening situations and how to intervene appropriately
Demonstrated leadership skills in critical thinking, effective communication, organizational skills, problem solving, and ability to make appropriate independent decisions
Working knowledge of applicable Standards of Practice
Computer skills required, prior experience with electronic medical record systems preferred
Demonstrated compassionate customer service and ability to meet the needs and expectations of patients and healthcare colleagues
Solid foundation in evaluating plans of care and reformulating as necessary
Qualifications:
Graduate of an accredited school of nursing is required; Bachelor of Science in Nursing is preferred
Registered Nurse (RN) license issued by the state of Connecticut is required
A minimum of five (5) years of experience as a Registered Nurse (RN)
Three (3) years of experience in critical care is preferred
A minimum of one (1) year of experience in a leadership role is strongly preferred
American Heart Association Healthcare Provider CPR certification or equivalent is required; if not currently certified, must obtain within three (3) months of employment
Additional specialty certification/training as required by the work area
