Position Summary: The Clinical Coordinator, under the supervision of a registered nurse and in accordance with physician orders, applicable law and regulation, accepted standards of nursing practice, payor requirements, and Interim HealthCare policies and procedures is responsible for supporting the day-to-day patient care services provided by the agency, whether directly or under arrangement.

Responsibilities include, but not limited to, making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care. Essential Functions: • Receives and coordinates incoming clinical calls, providing patients, physicians, and referral sources with basic information to assist in accessing appropriate services. • Provide effective communication regarding patient needs, physician orders, follow-up, or any interventions needed. • Acts as a contact telephonic support interfacing with referral sources, outside agencies, health professionals and field staff. • Communicates with patient and family regarding arrangements for the initiation and ongoing care/services. • Where permitted by law, receives verbal orders from physicians to initiate care and/or execute changes in the plan of care. • Works in conjunction with clinical staff to adhere to standards of practice for nursing and applicable law and regulations. • Actively participates as part of a high-performance work team to deliver exceptional patient/client service. • Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care. • Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. • Ensures services provided are ordered by the physician as indicated in the plan of care. • Recognizes and reports changes in the patient’s condition to the appropriate personnel. • Counsels and educates the patient and family in meeting healthcare related needs. • Follows accepted standards of nursing practice. • Participates in Quality Assessment and Performance Improvement (QAPI) program. • Participates in in-services or training, as requested. • Completes other assignments as requested and assigned. • Participates in the Office’s Emergency Preparedness program, as requested. • Accesses protected health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization.

Minimum Education & Experience Requirements: • Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which he/she practices • At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program At Interim HealthCare, we know that your loved ones deserve the very best – that’s why we attend to each individual’s needs in the comfort and dignity of their home.
