Job Summary

The Clinical Coordinator functions as a practice and program leader for pharmaceutical care activities, overseeing the clinical pharmacy program to meet all regulatory requirements, accreditation standards, and best practices. The incumbent is responsible for the direct supervision of clinical pharmacists; direct supervision may also include any combination of pharmacy technicians and pharmacy learners who work within the scope of responsibility.

The incumbent assists with maintaining departmental standards, internal and external reporting requirements, accreditation standards, and pharmacy laws and regulations. The Clinical Coordinator supports the departmental objective to have qualified clinical pharmacists accountable for delivery of optimal care and medication therapy to patients with a broad range of disease states as well as for upholding the centralized and decentralized medication use system. The incumbent ensures that clinical pharmacists proficiently provide clinical pharmacy services and integrated operational services in collaboration with pharmacists, pharmacy technicians, physicians, nurses, and other personnel.

The incumbent assists with drug utilization management and actively tracking and benchmarking outcomes of the pharmacy programs to assure operational, clinical quality, and financial efficiency. The Clinical Coordinator actively identifies practice related issues that require evaluation and facilitate improvement projects, quality initiatives, or healthcare provider education as needed to advance practice. The incumbent ensures medication use is aligned with evidence-based best practices and regulatory standards by monitoring processes and workflows, analyzing data, implementing interventions, and evaluating the effectiveness of those interventions. Responsibilities also include monitoring and documenting progress towards meeting departmental goals as well as providing guidance and education to staff on departmental priorities and projects.

The Clinical Coordinator also facilitates the development of student, resident, and pharmacist training experiences and competencies.

All responsibilities are accomplished in collaboration with the pharmacy leadership team under the direction of a Pharmacy Manager or Director.

Duties and Responsibilities

Supervision and Optimization

  • Oversee day to day operations within scope of responsibilities, including clinical pharmacy program and medication utilization to assure activities are aligned with patient care needs, evidence-based best practices, and regulatory standards

  • Supervise and lead the provision of safe, timely, efficient, equitable, effective, and patient-centered pharmacy services

  • Serve as the liaison to coordinate compliance and regulatory issues for areas of responsibility

  • Enforce clinical pharmacy workflows and practice expectations

  • Manage current electronic health record (EHR), technology, and automation for optimum value

  • Review reports for medication errors, adverse drug events, and/or ineffective service

  • Develop and/or maintain policies and procedures

  • Identify opportunities to redesign systems of oversight to maximize performance

  • Track and evaluate pharmacy programs for operational, quality, and financial efficiency, and routinely benchmark against local and national best practices

  • Oversee data collection activities that may be performed by pharmacy learners or clinical pharmacists

  • Identify practice related issues that require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice

  • Design and implement new pharmacy workflows, quality improvement changes, and continuous quality assurance initiatives to advance the organization's medication-use systems and promote optimal patient outcomes

Human Resource Management

  • Participate in the recruitment, interviewing, and hiring of staff

  • Evaluate performance of assigned personnel

  • Hold employees accountable to standards of the department and organization

  • Identify and address personnel issues in a proactive and timely manner, with appropriate follow-up to improve performance

  • Manage employee leaves of absence and family medical leaves

  • Ensure staff competency via training, competency assessments, and other means

  • Oversee the development, documentation, provision, and preparation of training information, ensuring that materials are updated and prepared to meet the training needs of clinical pharmacists

  • Maintain a training and competency program for clinical pharmacists that meets accreditation standards

  • Work with staff on an ongoing basis to attain their professional development goals by directing, coaching, and supporting

  • Develop staff to be eligible for and to attain available professional certifications and recognition of knowledge, skills, and abilities

  • Lead initiatives to identify gaps in training knowledge and areas for staff performance improvement

Project Planning and Leadership

  • Participate in the development of short and long-term goals of pharmacy services in consideration of its mission, vision, values, core purpose, and strategic plan

  • Participate in execution of the pharmacy strategic plan and assigned goals

  • Provide input and assist in the development of effective budgeting and utilization of resources to achieve agreed upon goals and objectives

  • Develop and maintain an expertise in change management tools and theories

  • Participate and/or lead, as pharmacy representative, organization quality improvement initiatives/projects

  • Provide departmental leadership for medical staff committees/meetings, hospital groups/task forces, and approved external programs/organizations

  • Assure pharmacy is an integral part of the health-care delivery system and facilitate enhancement and expansion of pharmacy services/programs

  • Serve as a pharmacy team liaison to interdisciplinary team members for areas of responsibility

  • Participate as a member of pharmacy department in multidisciplinary team and committees

Direct Patient Care (staffing)

  • Work under the responsibilities outlined in the Clinical Pharmacist position description up to 30%

Miscellaneous/Special

  • Additional duties as assigned

Job Qualifications

Education

  • Graduate of an ACPE-accredited School of Pharmacy or equivalent

  • Pharm.D. degree preferred

Experience

  • Completion of postgraduate year one (PGY1) pharmacy residency program required

  • Clinical pharmacy experience in a hospital setting required

  • Pharmacy leadership experience with supervisory experience preferred

Licenses/Certification

  • Currently licensed to practice in the state of California

  • Current Board of Pharmacy Specialties certification

  • Knowledge of state laws, federal regulations, and accreditation standards pertinent to pharmacy

  • Knowledge of pharmacy operations, clinical practice, and regulatory compliance

  • Knowledge of healthcare information systems and pharmacy automation and technology

Base Pay Scale: Generally starting at $73.00 - $98.00 per hour

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
