What will I do?

Our Clinical Coordinator LPN/RN for Assisted Living:

Under the direction of the Administrator, The LPN/RN Clinical Coordinator is responsible for oversight of clinical services and nursing care of Enhanced Services (assisted living). The LPN/RN Clinical Coordinator manages nursing supervisors, registered medication aides (RMA), certified nursing assistants (CNA), and other team members as assigned. You will use your clinical expertise and knowledge to effectively delegate and assign clinical responsibilities and provide oversight in the delivery of quality and consistent care.

Your leadership will be focused on ensuring a strong and survey-ready team, providing the best care for residents and the best support for one another. You will also demonstrate compliance and promote the same with all team members. You will serve as a member of the quality assurance and performance improvement (QAPI) committee and interdisciplinary team meetings.

You will be responsible for the oversight and execution of plan of care interventions responsive to risk-related events such as falls, skin conditions, and other clinical care areas. You will work community-wide with leaders to identify, address, and resolve the concerns that may impede us from meeting our goals. You will professionally represent WCCB with regulatory agencies and representatives, as well as with the family members and guests of residents.

Primary Responsibilities:

● Coordinates the direct care staff schedule to ensure it meets licensing requirements, organizational needs, and effective time management. Assists in finding coverage or providing resident care to ensure consistent clinical oversight of Enhanced Services residents.

● Performs frequent/regular walking rounds throughout the shift for the purpose of ensuring team members are adhering to policies/procedures/regulatory requirements, have the necessary resources (medications, equipment, and supplies), and are providing quality resident care.

● Provides quality oversight of processes such as admissions, discharges, risk management incidents, medication management, etc.

● Performs audits on a routine and ad hoc basis to survey compliance.

● Participates in or leads quality assurance and performance improvement projects, which includes staff development and training for direct care staff.

● Mentors and Coaches the LPN Supervisors to manage appropriately.

● Assists LPN Supervisors to resolve any issues that arise in a timely manner, and ensures updates are shared with other departments for problem resolution.

● Completes performance evaluations and manages incidents and disciplinary actions with LPN Supervisors and collaborates with the Administrator regarding any disciplinary actions prior to presenting to the team member.

● Identifies staff development needs of team members and provides recommendations to the Administrator.

● Serves as the liaison for team members, residents, family concerns and escalating them to the Administrator if collaboration or further resolution is needed.

● Participates in interdisciplinary team meetings (such as Huddle, Risk, Process Meetings, QAPI) and serves as an assisted living representative at campus meetings.

● Serves as a liaison between the in-house and community primary care practitioners for resident care needs.

● Assesses and suggests process and policy improvements to continually improve the quality of resident care.

● Empowers residents to self-direct their own care and support this need by coaching and training teams to recognize individuals.

● Provides input to the interdisciplinary team in assessing residents for an appropriate level of care prior to discharge to The Hoy Center or hospital and monitors residents with a change in condition or acute illness closely.

● Provides appropriate input into service plans to incorporate residents' needs and preferences, family's desire for care and interdisciplinary input.

● Assists with creating and disseminating resident, family, and staff communications.

● Observes community protocol to prevent, detect, investigate, and report abuse.

● Participates in continuing education programs to maintain competency and improve knowledge and skills.

● Provide guidance related to resident changes in condition and resident emergencies to direct care staff and security

● Performs all other duties as assigned.

Qualifications:

● LPN or RN license in the State of Virginia

● At least three years of clinical and managerial experience in long-term care or Assisted Living.

● Certification or evidence of continuing education in gerontological nursing and nursing administration in long-term care or Assisted Living is recommended.

● Must be proficient with software applications and systems to facilitate communication and resident care such as electronic medical records, email, word processing, etc.

WHAT YOU WILL NEED TO BE SUCCESSFUL:

● LPN or RN license in the State of Virginia

● At least three years of clinical and managerial experience in long-term care or Assisted Living.

● Certification or evidence of continuing education in gerontological nursing and nursing administration in long-term care or Assisted Living is recommended.

● Must be proficient with software applications and systems to facilitate communication and resident care such as electronic medical records, email, word processing, etc.
