The Clinical Coding Specialist is responsible for monitoring and performing accurate diagnosis coding. You will monitor clinical documentation and ensure accurate coding is completed in a timely manner and that records are coded to the highest degree of specificity. You will act as a qualified coding resource person for clinicians and other coding staff. You will monitor regulations and work within the ICD 10 guidelines and regulations. You will provide clinical staff education related to coding and coding guidelines.


  • Receives automated and manual tasks alerting to the need for coding of patient assessments.

  • Marks tasks as "In Process" as a way to acknowledge and reserve the assessment to prevent premature OASIS submission.

  • Adds correct ICD 10 codes to medical record based on review of clinical data and communication with clinician.

  • Makes corrections based on collaboration with clinician.

  • Identifies viable primary and secondary diagnosis necessary for accurate plan of care.

  • Follows sequencing and reporting procedures mandated by Medicare and other payers for completion of coded data.

  • Creates Episode Analysis report and communicates findings with Clinical Manager.

  • Maintains up to date OASIS assessment and home health coding knowledge.

  • Ensures review time from date received does not exceed 24-48 hours during normal business hours.

  • Meets minimum productivity requirements of 10 12 reviews per day upon completion of an appropriate orientation period.

  • Contributes to team effort by accomplishing related results as needed.

  • Performs other duties as assigned

  • Honor patient's personal and property rights

  • Understands all rights afforded to patient's and acts accordingly


  • Two years of home care industry experience.

  • Licensed Registered Nurse in Ohio and/or West Virginia

  • Previous Home Health coding experience preferred.

  • ICD-10 certification

  • In depth knowledge of the latest OASIS version, Medicare guidelines and home health standards.

  • Proficiency with Microsoft Office (Word, Excel, PowerPoint, Outlook).

  • Certification by a HCS-D or AAPC and HCS-O or COS-C program required, or must be certification eligible and obtain certification within six months of accepting the position.

VIEW JOBS 9/6/2019 12:00:00 AM 2019-12-05T00:00 Summary: The Clinical Pharmacy Specialist works independently under the Director of Clinical Pharmacy Services and keeps them apprised of trends/problems associated with their assigned areas. The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through direct interaction with patients, providers and interdisciplinary team. The clinical pharmacy specialist (CPS) works independently under their scope of practice to directly care for patients. A CPS serves is a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. The Clinical Pharmacy Specialist functions as part of the interdisciplinary team to accomplish missions and goals of the center. Specific responsibilities and services include, but are not limited to: * Designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; * Helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; * Per approved scope of practice, prescribes medications, performs physical assessments; and orders laboratory and other tests to help determine efficacy and toxicity of medication therapy; * Obtains medication histories from patients, completes medication reconciliation and summarizes significant findings in the medical record and/or to the provider as appropriate; * Reviews patient medication regimens for cost effective drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required; * Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed; * Monitors for and reports drug errors, adverse drug reactions, allergies, patient compliance issues and documents findings per center policy; * The pharmacist has expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is able to make dose adjustment recommendations based on objective laboratory findings; * Reviews and evaluates requests for non-formulary and restricted drug consults for appropriateness in compliance with established criteria where applicable and provides therapeutic alternatives as necessary; * Documents clinical interventions in electronic health record and pharmacy database in a timely and professional manner as appropriate; * Promotes and monitors compliance for established drug therapy policies. * Works with providers to ensure compliance with center initiatives; * Responds to emergencies and offers assistance to Code Team as appropriate; * Fulfills the role of a Consultant Pharmacist when requested, as workload dictates; * Participates in the training and in-servicing of clinical staff, pharmacy technicians, and pharmacy residents as required; * Effectively counsels patients and family members in the appropriate use of medications and medical devices; * Acts as point-of-contact for resolution of time-sensitive nursing/pharmacy issues; * Participates in the center and pharmacy service quality improvement programs. Pharmacists assigned to this position must demonstrate the following: * Ability to communicate orally and in writing to persuade/influence clinical and management decisions. * Expert understanding of regulatory and quality standards for their program area. * Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in area of clinical expertise. * Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. * Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment, interpretation of laboratory and other diagnostic parameters. Qualifications: * Post-graduate Year Two (PGY2) Residency training in a Geriatrics Residency with experience providing direct Geriatric patient care with scope of practice responsibilities similar to those required by this position * OR Post-graduate Year One (PGY1) Residency in general clinical pharmacy (e.g. Pharmacy Practice, Managed Care Pharmacy Practice, Primary Care/Ambulatory Care) with Board Certification in Geriatrics (BCPS) or Board Certification in Ambulatory (BCAP) or Pharmacotherapy (BCPS) and experience providing direct Geriatric patient care with scope of practice responsibilities similar to those required by this position * OR 3 plus years of experience providing direct Geriatric patient care with scope of practice responsibilities similar to those required by this position and eligibility for board certification within one year Stonerise Healthcare LLC Charleston WV
