The Department of Neurology is looking for a Clinical Case Manager who will work collaboratively with interdisciplinary staff internal and external to the organization. The Clinical Case Manager will participate in quality improvement and evaluation processes related to patient satisfaction and patient rights and serve as an information resource for the Fixel Institute of Neurological Diseases (FIND) in the Department of Neurology.

About This Role

As a Clinical Case Manager, you will promote and facilitate developing a specific treatment plan or strategy that reflects the therapeutic intervention that addresses the patient(s) need. You will provide patient navigation, especially for patients moving among different health care levels (inpatient, outpatient, home-health, etc.). You will provide education and support programming; assist with medications, transportation and advance directive; outreach and coordinate with other community resources and agencies and community-level advocacy on behalf of patients and families.

You will be responsible for the referrals seen and evaluated in the Memory Disorders Clinic and for the Neuromuscular at the program for Movement Disorders and Neurorestoration. You will subscribe to the Silver Alert Listserv, and when a Silver Alert is issued, you will follow-up in accordance with the Silver Alert Guidelines.

You will respond to emergency needs of patients and families by providing interventions necessary to stabilize, improve or resolve crisis situations. This role will require you to remain manage crisis situations effectively and to problem solve under pressure to help our patients and families. You will want to be comfortable communicating with appropriate parties as issues arise and be able to identify priority and key issues in complex situations.

You will document social work services rendered by documenting in the electronic medical record (EMR) and maintaining accurate records and submitting other necessary documentation as needed in this role. You will help us with research initiatives by inputting data into a tracking system.

You have the opportunity to conduct annual service provider and caregiver memory training, participate in annual training for Elder Affairs, and attend Memory Disorders quarterly meetings. You will attend memory disorders clinics at FIND and provide annual reporting on the number of specialized training programs developed, updated, and/or provided for the following groups: caregiver groups/organizations and service providers.

You will submit quarterly and annual reports for Elder Affairs as well. You will attend conferences and meetings as a representative of the Norman Fixel Institute of Neurological Diseases and participate in committees and focus groups.

Expected Salary:

$45,000 - $55,000; commensurate with qualifications and experience.

Minimum Requirements:

Master's degree in an appropriate area; or a bachelor's degree in an appropriate area and two years of relevant experience.

Preferred Qualifications:

Knowledge of the professional principles and skills used in the practice of social work; professional empathy in dealing with families; sense of professional identity while collaborating with co-workers; awareness of community resources and programs for the disabled and elderly; appropriate professional judgment, courtesy and tact.
