Summary: Women's Substance Abuse Services Clinical Case Manager will provide individual and group counseling to residents. The Clinical Case Manager will carry a case load of up to eight clients. This individuals clients will be primarily dual diagnosed or individuals who have been identified as high risk clients.

Schedule: Monday - Friday 11:00 AM - 7:00 PM (40 Hours)

Essential Duties & Responsibilities:

  • Complete Clinical Intake within the first week of admission

  • Complete Treatment Plans within first 2 weeks of admission and update as needed

  • Be proficient in the use of the electronic Medical Record for each case

  • Meet weekly with assigned individuals

  • Participate in Clinical Supervision

  • Input entries into the Medical Record must be in line with the service line procedure

  • Participate in the on-call Rotation

  • Expand knowledge and participate in trainings

  • Understand DPH regulations and CARF standards as applicable

  • Provide transportation as needed

  • Establish contacts with DCF, Guardian, doctors and therapists etc. to involve all parties in the treatment process

  • Make outside referrals as needed

  • Attend weekly staff meeting

  • Provide weekend holiday and emergency coverage

  • Other duties as assigned.

Required Education/Experience:

  • Bachelor's degree Preferred or clinical licensure preferred

  • A minimum five years' experience in the field

  • Individual and group counseling experience

  • Must have MA driver's license

Required Skills/Knowledge:

  • Strong written and verbal skills

  • Ability to maintain records accurately and efficiently

  • Capable of responding to crisis situation

  • If recovering, two years continuous sobriety required

  • Ability to work independently as well as part of a team
