Welcome to Connections for Kids Come join our growing team Connections for Kids is one of Maines leading mental health agencies We are a small group of talented individuals united by a common path to purpose and have been making a difference in the lives of children and families in Maine for 26 years Position Targeted Case Manager TCM Compensation 20 24Hr Status Full Time 40 HrsWeek Create your own schedule Scheduling usually falls within daytime hours but can include availability for afternoons evenings or weekends at your discretion Location On the Road Covering Cumberland & Androscoggin Counties Job Description We are seeking an amazing candidate for our Targeted Case Manager position to work with children and families in the Androscoggin and Cumberland County communities Our Targeted Case Managers take the leading role in supporting family connections and assisting with community resources; they work with children with mental illness behavioral challenges or developmental disabilities Working as a Targeted Case Manager for CFK you will assess client needs and develop implement and coordinate their individualized plan in collaboration with family or other supports Qualifications Must have a Bachelors degree in social work or a related field to qualify for state licensure
