About the Role:
In the role of Clinical Care Coordinator, you will be a critical liaison between our clinicians, care team and Equality Health members. You will work closely with our clinicians to register patients virtually or in person, schedule visits, prep for in home or telehealth visits and support our clinical team through completing follow up orders, highlighting issues and gaps and working with our members to coordinate their next visit and follow-up care. This may include communicating and working with Equality Health care team members, external PCP offices, coordinating pharmacy services, and helping manage the durable medical equipment (DME), home health care, imaging, and laboratory testing needs of our members. Your goal is to make our team more efficient, make the process for our members smooth and help ensure they can adhere to their plan of care and better able to get the care they need.
Candidate must reside in Rio Grande Valley area
Responsibilities:
Registering new members and scheduling new and follow-up visits
Helping intake members for a telehealth visit
Working with our members to help log into their telehealth visits or send reminders for in home visit with nurse practitioner (NP)
Order management/care coordination
Working with NP and home health partners to coordinate DME and skilled nursing services
Coordinating lab testing and imaging and to tracking results
Notifying members of lab results and helping to coordinate follow care at the direction of the clinician
Coordinating with Equality Health NP and PCPs on medical record collection, information gathering, setting up and confirming appointments, and submitting outbound communications from DMG
Triage support
Helping triage clinical and non-clinical issues from members to the appropriate teams (care team, nurse practitioner, pharmacist, social worker, practice)
Follow-up with members post ER and Dispatch Health visits to assess for status and follow up need
Serving as an advocate for our members patient education when they call DMG for care coordination issues both through answering and returning phone calls
Supporting members to improve management of their chronic condition(s), and coordinating with Equality Health care team to align care
Monitor, follow through of recovery action plans, follow up appointments and self-management plans
Travel throughout Rio Grande Valley area <10% via personal automobile
Required Knowledge, Education & Experience:
High school diploma or GED equivalent; Associate degree in a related field of study is preferred
Minimum two (2) years of work experience as a Medical Assistant for a primary or specialty medical practice
Graduate of a Medical Assistant training program accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP), the Accrediting Bureau of Health Education Schools (ABHES), or by any accrediting agency recognized by the U.S. Department of Education
Demonstrated knowledge of various EMR systems and case management
Demonstrated knowledge of phlebotomy, anatomy, physiology, and medical terminology
Superior verbal and written communication skills, and the ability to function in a caring and empathetic manner with patients and their families
Bilingual English/Spanish
Proficiency with Microsoft Office applications and web-based technologies
Experience with and comfortable using keyboards, laptops, tablets and other electronic and portable devices for documentation and telehealth visits
Highly Preferred Skills, Abilities & Qualifications:
Certified Medical Assistant or Registered Medical Assistant
Experience with Athena EMR system
Able to handle multiple activities or interruptions and work independently in a fast-paced, customer-facing environment
Demonstrated ability and willingness to treat patients with the utmost kindness and consideration in the most trying situations
Excellent attention to detail and organizational skills
Physical Requirements
On occasion, the individual may be required to lift up to 25 pounds independently, anything over 25 pounds with assistance.
Vision and hearing acuity.
Work involves walking, talking, hearing, using hands type, feel or operate objects and tools to include computers and office equipment.
Ability to sit in a stationary position for longer periods of time.
Must have the ability to perform the essential job duties with or without reasonable accommodation.
