Summary:
State of CT Registered Nurse

Provides clinical leadership and expertise in area of specialty and provides staff counsel. Collaborates with the manager in the development and implementation of education processes for surgical patients. Provides coordination of the planning process for surgical patients.

Responsibilities:

1.Provides clinical leadership and expertise in area of specialty. Counsels staff in conformance with Hospital policy.
2.Collaborates with the manager in the development and implementation of education processes for surgical patients undergoing or undergone surgery.
3.Evaluates staffing requirements and recommends changes as needed.
4.Provides input in the development of departmental policies and procedures. Maintains guidelines and protocols, and monitors adherence to protocols by provider.
5.Develops and implements patient education as appropriate to the specific patient population.
6.Works with multiple providers in giving care to surgical patients.
7.Triage's patients appropriately in the absence of the physician. Responsible for taking thorough and accurate histories on all new patients at time of surgeon consultation.
8.Ensures the appropriate maintenance of surgical equipment and medications. Ensures the practice remains compliant with medication storage and utilization, surgical equipment handling, cleaning, and sterilization procedures.
9.Assists with in-office procedures as requested.
10. Performs simple wound evaluations. Provides wound care dressing changes, suture and staple removal as directed.
11. Collaborates with physician, Home Care in-patient nurses and case coordinator to facilitate the care of patients with complex needs throughout the care process. Coordinates patient care needs pre-operatively with other members of the healthcare team.
12. In absence of an available provider, acts as a liaison with residents, other MD's and nurses, as needed to ensure continuation of patient care.
13. Assists with the implementation and evaluation of the after-care plan through a process with patient and family, linking patient with appropriate resources.
14. Oversees program compliance. Ensures the delivery of quality and cost effective care through problem solving and implementation of quality assurance/improvement measures.
15. Attends education as appropriate for specialty.
16. Fulfills all compliance responsibilities related to the position.
17. Performs other duties as assigned.

Summary: Develops, oversees, and monitors care management and coordination activities for the patients of the primary care/sub-specialty practice. Supports primary clinical teams with these efforts. Identifies the high acuity patient population and works directly to ensure care coordination. Collaborates with the Practice Managers and Medical Director (lead physician) to develop services and initiatives that best serve the needs of the patient panel and the primary care teams. Responsibilities: 1. Works with all clinical teams as a resource on care management of all practice patients to include the following: pre-visit planning workflow to ensure care completion prior to visit, and after visit review with patients; educates the patient about self-management tasks to gain greater control of their health status. 2. Works with IT resources to facilitate registry reporting at the site and documentation of EBC in searchable fields. 3. Collaborates with physician leadership to define quality measure/outcome reporting process, and develops protocols and point of care reminders using nationally recognized EBC measure/outcomes. 4. Serves as a resource to clinical staff and providers to establish quality goals using reports. 5. Manages assigned panel of high acuity chronic care patients. Develops relationship with patients as an integral member of the team. 6. Manages aspects of the patients care i.e. referrals to specialists, hospitalizations, ancillary testing, and other services. Provides follow up contact with patient to ensure compliance with recommendations i.e. medications, lab/x-ray, specialist visits, etc. 7. Responsible for being available to provide telephone advice according to protocol and handle urgent and emergent calls. 8. Responsible for working with the patients' care team to coordinate change readiness, needs assessment, and develop an individualized treatment care plan. Completes necessary documentation and pre-visit planning that is requested before visit. 9. Assists patients in setting goals for self-management tasks and reports abnormal findings to their physician team. Oversees the development, procurement, and adoption of patient self-management educational resources used by the primary clinical teams. 10. Assesses barriers when patients have not met treatment goals. 11. Collaborates with payer Case Managers for additional services when appropriate. 12. Develops list of medical supply and community resources available to patients. Maintains collegial relationships with the entities most frequently used. 13.
