The Clinical Advocate will assist patients with insurance, making appointments, testing, treatment, education, transportation, psychosocial needs and managing any other barriers that would prevent a patient from getting treatment in a timely manner.
The Clinical Advocate will collaborate with the physician and APN in the management of heart failure, post heart transplant, and VAD patients.
Responsible for evaluating and screening patients for a VAD along with the interdisciplinary team.
Plays an extensive role in patient and family education pre and post VAD implant.
Responsible for patient management in the outpatient HF clinic .
Actively participates in departmental quality improvement/data review, collects data required for performance improvement measures, follows all policies to maintain certification requirements, and interact with physicians and other staff as appropriate.
Has rotating on-call responsibility for VAD related emergencies and troubleshooting as needed.
Current and valid New Jersey RN license is required
Active BLS/CPR by American Heart Association is required
Minimum of 3-5 years of experience in cardiovascular nursing, cardiac surgical nursing, critical care nursing, cardiac transplantation, and/or care of patients with ventricular assist devices required
Knowledge of computers and database information systems required
