Position Description

As part of our Center for Service Coordination department, you will be a key stakeholder in supporting our members. Your primary role will be outreaching to our members post-discharge to ensure all of their healthcare needs are met. Identification of the appropriate programs and services available is critical to the success of our members.

  • Office base position located at 20414 NORTH 27TH AVE, Phoenix, AZ. Monday-Friday from 8am-5pm.*

Primary Responsibilities:

  • Outreach calls to our members / beneficiaries post facility discharge

  • Support the enrolled members / beneficiaries of our community partners

  • Collaborating with our PCP Network, physician offices and hospital facilities within Maricopa County

  • Support members with coordinating healthcare services

  • Arranging post-discharge appointments, transportation, and transition of our members across the continuum of care

  • Make appropriate referrals to the Customer Care team to support benefit, billing / payment and eligibility of our members

  • Assist with referrals / calls received by the Center for Service Coordination department, and complete community outreach and documentation

Required Qualifications:

  • HS Diploma or GED

  • 1+ years of customer service experience analyzing and solving customer problems with great communications skills

  • Experience working within the healthcare industry

  • Professional experience in a clerical or administrative support related role

  • Working knowledge of medical terminology to communicate with members / beneficiaries and providers

  • You will be asked to perform this role in an office setting or other company location, however, may be required to work from home temporarily due to space limitations

  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.

Preferred Qualifications:

  • Some college / undergraduate education

  • Medical Assistant (MA) certification

  • Experience working with Centers for Medicare and Medicaid Services, especially Medicare and Medicare Advantage programs

  • Knowledge of community resources

View our Covid-19 Resources Page at: https://info.uhg.com/ Primary Responsibilities: * Work with a Clinical team of a Pharmacist and Clinical Care Coordinator (Social Workers) to support member and provider engagement for Care Transitions, Medication Therapy Management, Referral Management, High Risk Member management, etc. * Work with hospitals, clinics, facilities, and pharmacies to capture pertinent information for patient outreach * Accurately manages multiple technology systems to track down pertinent data to prep charts for Pharmacist * Engage members telephonically for warm transfer to pharmacist for Comprehensive Medication Review or Medication Reconciliation * Engage members telephonically for Medication Adherence concerns, identify barriers if any * Engage members telephonically to connect them back to Primary Care Providers * Engage members telephonically to notify of out of network service use * Work closely with Social Worker/Pharmacist to help ensure that patients have comprehensive and coordinated care * Accurately manages multiple technology systems simultaneously * Accurately document interactions and appropriately follow all processes and procedures related to engagement * High telephonic proficiency, with a focus to meet daily assigned contact metrics and ability to multi - task * Build rapport and establishes trusted relationships with members via daily outbound and inbound calls * Provide excellent customer service * Requirements: To be considered for this position, applicants need to meet the qualifications listed in this posting.
