The Patient Advisory team is the first point of contact for our immediate and extended communities, PAN RN's are the key to making sure our patients are cared for 24/7. EDUCATIONAL REQUIREMENTS
College Diploma in Nursing
CERTIFICATION & LICENSURE REQUIREMENTS
Registered Professional Nurse (RN) License Illinois upon hire and Registered Professional Nurse (RN) License Indiana, Iowa, Ohio, and Washington within 1 year.
EXPERIENCE REQUIREMENTS
Two (2) years of clinical RN experience (inpatient, outpatient, or ambulatory settings)
SKILLS AND KNOWLEDGE
Ability to use clinical knowledge, reasoning, and nursing processes to appropriately manage and direct patient care based on level of acuity and need. Advocates, directs, and provides guidance for the safest and most appropriate level of care by obtaining pertinent health information, conducting thorough review of patient history, and applying clinical knowledge to reported concerns. Uses nursing process to appropriately evaluate and assess the needs of the patient and following department outlined procedures. Must possess strong ability to effectively communicate and maintain professionalism with patients, physicians, and other clinical or clerical personnel. Works well in conjunction with case managers, social work services, and outside facilities. Able to incorporate, adapt, and drive change processes. Must maintain ability to stay organized, utilize resources, and provide solution-oriented care and teamwork. Basic PC and data skills are required, additional knowledge of Microsoft, Epic, and technical applications are a plus.
ESSENTIAL FUNCTIONS:
Assesses and triages patient concerns via telephone using embedded adult and pediatric protocols.
Documents all telephone encounters received/initiated within Epic.
Completes appropriate EMR documentation and follow up including medication reconciliation.
Provides medication management assistance by renewing, refilling, cancelling, or transcribing medication orders as appropriate within departmental guidelines.
Provides education and counseling to patients, families, and groups.
Facilitates care coordination between patient and provider during 24/7 operations.
Communicates with outside facilities when appropriate.
Assists with department tracking and reporting events as requested.
Provides additional support to other Carle clinical team members when needed.
Acts as a preceptor and supports the development and retention of other staff.
Facilitates and provides education presentations when given the opportunity.
Primarily providing telehealth triage for acute needs for Carle Health, Health Alliance, and additional contracted health systems through daytime and after-hours to facilitate and maintain patient safety and care.
Maintains working knowledge of specialized processes including but not limited to: Receiving and communicating critical lab value reporting, heart monitor alerts, hospital discharges, AMS patients, Health Alliance ask a nurse portal, pre-procedure calls and rescheduling, patient and provider appointment cancellations, and other miscellaneous tasks as outlined within onboarding and orientation.
