The RN Care Coordinator communicates and collaborates closely with providers and other members of the health care team to provide consistent quality care for patients. Working with a high degree of autonomy, the RN Care Coordinator accepts responsibility and accountability for assigned patients as a clinic nurse and care manager to plan implement and evaluate the care for patients.
This position requires excellent teaching and/or communication skills (individual/group) which will be essential in the interview (intake) process with patients. The provider will design a prescription follow up plan and the RN Care Coordinator needs to fully understand the plan in order to reinforce it with the patient. The RN Care Coordinator is directly accountable and responsible to the consumer for the quality of nursing care rendered. Performance of duties must sincerely and positively reflect consideration, concern, and inspire confidence when interacting with patients, visitors, providers or fellow employees.
The RN Care Coordinator is responsible for pertinent patient information regarding the plan of care the patient receives. Coordinating communication with nurses and providers is a high priority. The RN complies with the State Practice Act, hospital and nursing philosophy standards while performing job duties.

Required Qualifications:
Graduate of an accredited school of nursing
Upon hire: Montana Registered Nurse License
Upon hire: National Provider BLS - American Heart Association
