Midland Care is hiring a Clinic Nurse for our Topeka PACE Center.
Summary
In this position you will play a key role in delivering high-quality care to our participants in the Program of All-Inclusive Care for the Elderly (PACE). Responsibilities include direct patient care, medication administration, and collaboration with the healthcare team to ensure the well-being of our participants.
Midland Cares Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement to stay in their own homes and communities.
Pay Rate starting at $26+/hr for LPN | $32+/hr for RN
$2,500 Sign on Bonus for Full-time LPN
$3,000 Sign on Bonus for Full-time RN
Essential Duties & Responsibilities
Assures excellent customer service is provided to all participants.
Completes nursing responsibilities including but not limited to monthly medication reconciliation, quarterly medication review with participants, and timely completion of required Functional Capacity Screens and Health Services Plans as designated by regulation.
Promotes and maintains the health of participants through evaluation of care needs and provides teaching and instruction to meet care goals.
Provides skilled nursing care, by following identified protocols, infection control standards and prescribed treatments.
In conjunction with RN, identifies priorities on a daily basis and provides needed confirmation of critical activities before leaving the site.
Assists in the evaluation of medication compliance and the effectiveness of prescribed medications.
Assists in the transcription of medication orders and communication with pharmacy providers.
Attends and participates in daily meetings, and scheduled participant care meetings including IDT as requested.
Confers with the Interdisciplinary Team for Plan of Care adjustments and enters changes in Electronic Medical Record as agreed upon by the team.
Maintains confidentiality of all participant events, discussions and records.

Qualifications
Required: Current unrestricted State of Kansas License to practice as a Licensed Practical Nurse or Registered Nurse; Valid Kansas Drivers License. At least one year of experience as an LPN or RN, preferably in home or community health; must be able to relate well with seniors and their families; Experience working in an interdisciplinary team setting is desired; Computer literate. Must possess valid Kansas Drivers License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organizations insurability standard as defined by Midland policy.
