Position Summary:

The Clinic Nurse assumes responsibility for the delivery of patient care utilizing the nursing process, under the supervision of a physician. This position requires population specific competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires.

Essential Functions:

The Clinic Nurse responsibilities include, but are not limited to, the following:

  • Contributes to the assessment/reassessment of patients

  • Participates in the development, implementation and evaluation of appropriate patient care interventions

  • Monitors for signs and systems related to abnormal lab values, and deviation from baseline assessments

  • Reports changes in patient status to the physician and initiate any new orders from the physician as directed

  • Promotes patient satisfaction and ensures patient confidentiality at all times.

INTEGRIS Family Care Clinics:

  • Participates in charge management to ensure chart accuracy with each visit.

  • Documents patient information in the medical record accurately and timely.

  • Reports suspected abuse and refers patient/family to appropriate health or social resource.

  • Performs Venipuncture, EKG, Waived Testing, Bone Density, X Ray, and other procedures as assigned by the physician, manager or clinical supervisor. Performs sterilization of equipment after a procedure is performed.

  • Gathers and documents information about patient condition, which includes lab values, pain levels, patient education needs, nutritional status and potential complications.

  • Contributes to the assessment/re assessment of patients.

  • Monitors and documents patient status.

  • Participates in the development of the plan of care, including implementation and evaluation of appropriate patient care interventions which may include medication administration.


Reports to the department director, business office manager, or clinical manager/weekend supervisor.

Environmental Conditions:

Potential for exposure to infections and communicable diseases, blood and body fluids, electrical equipment, chemicals. Must follow standard precautions.

Home Health: Exposed to inclement weather conditions while driving to and from patient homes. Frequent exposure to extreme temperature changes, poor environmental aspects such as poor lighting, poor ventilation, excessive heat/cold, stressful patient/family situations, unclean environment, body odors, pet odors, cigarette smoke. Some homes are unsanitary and are rodent infected.

VII. Required Competency Test(s)


  • LPN


  • Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state.

  • Kansas RN or LPN license is required for INTEGRIS Family Care clinic in Caldwell, Kansas

  • Previous work experience preferred

  • BLS and/or ACLS as required by department.

  • Must be able to communicate effectively in English (Verbal/Written)

IMG/ICP Float Pool Positions Only:

  • This job requires the incumbents to operate an INTEGRIS owned vehicle OR personal vehicle (non INTEGRIS owned) and therefore must have a current Oklahoma State Drivers License as well as a driving record which is acceptable to our insurance carrier.

IMG/ICP staff for the INTEGRIS Family Care Kansas Clinic:

  • This job requires the incumbents to operate an INTEGRIS owned vehicle OR personal vehicle (non INTEGRIS owned) and therefore must have a current Kansas State Drivers License as well as a driving record which is acceptable to our insurance carrier.

Creates a welcoming and engaging environment as the first point of entry for NorthCare and initial engagement in services at NorthCare. Provides recovery-focused case management, crisis diversion and rehab services to meet the needs of those seeking services, or assigned to case load. DUTIES AND RESPONSIBILITIES (includes but not limited too): Provides 55% of time in direct consumer service. Part time limited to 29 hours per week. Provides services that are welcoming, engaging and accessible. These services are recovery focused, strengths based, culturally competent, consumer centered, and co-occurring capable. Works as part of treatment team to meet the needs of the consumer in a timely and thorough manner. Interacts with consumers in a respectful, genuine and professional manner. Provides initial triage and assessment by phone or in person to persons seeking services which includes gathering of information about the symptoms, behaviors, functioning, history, needs and preferences to assist in decision-making process. Facilitates referral of consumers into the appropriate agency program(s) or to another community provider. Obtains the necessary releases and consents and advises the consumers about their rights and responsibilities. Consults with other program staff, as needed, to facilitate referrals. Completes fee agreements and necessary documentation associated with the fee agreement paperwork. Becomes knowledgeable of types of insurances, verification and different contracts within the agency. Schedules intake and medication clinic appointments for consumers referred into the agency. Makes follow up calls to engage the consumer and facilitate their recovery process. Completes discharge paperwork as required by agency policies. Provides case management services for consumers, assisting them in obtaining needed benefits and resources, including, but not limited to, housing, employment, medical, recreation, social and family supports, nutritional information and medication. Establishes linkage with inpatient treatment teams and provides support services to consumers who are hospitalized and being discharged from hospitalizations. Must be knowledgeable of community resources and maintain positive working relationships with other agencies. Contacts all consumers on case load at least once a month to monitor progress toward goals and objectives, determine needed resources and assistance, and to provide support and encouragement in the recovery process. Provides transportation as required to various sites in the community. Helps the consumer to recognize and utilize their own strengths, skills, talents, and abilities to increase independence and self-sufficiency in the community. Facilitates development and/or maintenance of relationships with family, friends, neighbors and significant others in the community. Provides individual rehab and teaches educational, recovery-focused rehab classes to improve the skills and abilities of the consumers to live interdependently in the community, improve self care and social skills, and to promote lifestyle change and recovery practices. Assists the consumer in the acquisition, development and maintenance of vocational skills as well as vocational opportunities as appropriate. Participates in weekly program meeting. Initiates and/or participates in treatment team staffing meetings as appropriate. Maintains appropriate staffing documentation. Assists consumers with the development of their treatment and recovery plan, and coordinates involvement in services. Under the close supervision of the Supervisor and other management staff and/or with the assistance of an LBHP Clinician, provides crisis management services. This may include face-to-face or telephone interviewing of the consumer; assessment of a consumers lethality to self or others; assessment of a consumers available support systems; and assistance in arranging for appropriate intervention, including hospitalization. In the cases where a potential for harm exists, will consult with management staff or available therapist to develop a safety plan with the consumer. Maintains clinical records in a timely manner, as required by agency policy, funding sources and accreditation authorities. Completes all required forms and maintains same in a manner consistent with requirements. Knowledgeable regarding: NorthCares electronic record system and public folders; ODMHSAS Services manual, contract requirements, and benchmarks; National Outcome Measures (NOMS), Medicaid rules and regulations. Participates in trainings/workshops related to triage, assessment, crisis intervention, case management and in expanding knowledge base in recovery focused mental health services and substance abuse services.
