What you'll do:

The LPN at Thome PACE works in a close collaborative relationship with the Providers and Registered Nurses to provide integrative care to frail elders with complex needs in order to enhance functional capacity, encourage autonomy, and assure coordination of care and services.

  • Provides medical care and oversight with the Interdisciplinary Team, including wound care, medication administration, immunizations, acute clinical needs, participant and caregiver education, and service coordination.

  • Collaborates with participants, families, and the Interdisciplinary Team to develop care plans

  • Maximizes participant functional capacity by promoting autonomy in care

  • Monitors participant progress and notifies team of changes in status

  • Maintains accurate and timely records per Thome PACE policies

  • Advocates for participants and resolves concerns

  • Provides after hours on-call assistance via phone triage on a rotating basis

What it takes

  • Licensed Practical Nurse with current Michigan licensure

  • Must possess a current State of Michigan driver's license and maintain an acceptable driving record

  • Must have reliable transportation available daily basis

  • BLS for healthcare providers or CPR Certification
