Position Summary The Cleft Palate Clinic Nurse Coordinator is responsible for coordinating patient care delivered through the Cleft Palate Outpatient clinic and serves as a liaison with the inpatient team for patients who are hospitalized. The Nurse Coordinator participates as a member of the Cleft Palate multidisciplinary team and assumes responsibility to ensure the smooth and efficient delivery of healthcare services across the continuum of services.

The Nurse Coordinator collaborates with the team to organize healthcare needs/services, analyze and records data accurately in health care records and registry. Maintains compliance with applicable state, federal and national regulations and the ACPA-CPF Guidelines. This position supervises two staff members, and works closely with a wide range of contracted clinicians, social services, and clinic staff. # Required Minimum Knowledge, Skills, and Abilities (KSAs) Current license to practice as a registered nurse in the State of Maine.

Bachelor#s degree preferred; RN, PA NP with related clinical experience. Maintain licensure and certification of professional stature. Strong verbal, written and interpersonal skills, materials.

Ability to work, in group situations and with the public. Ability to organize and prioritize work to meet deadlines. Ability to work under stress with numerous interruptions, distractions and changing priorities. Strong software and computer skills, including Microsoft Office (including Power Point, Access, and Excel), database and website utilization and/or development.

Fundamental Components: Through the use of clinical tools and information/data review, conducts an evaluation of member's needs and benefit plan eligibility and facilitates integrative functions as well as smooth transition to Aetna programs and plans.Assessments will take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Reviews prior claims to address potential impact on current case management and eligibility. Assessments will include the members level of work capacity and related restrictions/limitations. Application and interpretation of disability criteria and guidelines, applicable policies and procedures, regulatory standards and disability benefit plan to determine eligibility and integration with available internal/external programs. Using holistic approach assess the need for a referral to clinical resources for assistance in determining functionality. Consults with supervisor and others in overcoming barriers in meeting goals and objectives, presents cases at case conferences for multidisciplinary focus to benefit overall claim management. Utilizes case management processes in compliance with regulatory and company policies and procedures. Supports, integrates and executes the IHD process. Utilizes assessment techniques to assess for integration opportunities and determining work capacity. Utilizes l interviewing skills to ensure maximum member engagement in disability process and a timely RTW.
