Clinic Care Coordinator-Lpn (Licensed Practical Nurse)

JOB SUMMARY/PURPOSE Functions to serve as an advocate, educator and coordinator of services for at-risk patients. DEPARTMENT DESCRIPTION The Breast Center team at Samaritan Health Services provides comprehensive breast health care services and quality care to patients with varying levels of breast health concerns. The team facilitates continuity of care between the hospital, clinical programs, and ancillary services. EXPERIENCE/EDUCATION/QUALIFICATIONS Current unencumbered Oregon LPN license required. One (1) year experience in a healthcare setting required. Two (2) years LPN experience preferred. Healthcare Provider Level BLS required within 90 days of hire. Experience in the following preferred:# Outpatient, ambulatory care and/or community healthcare. Care coordination. A primary care related setting. Chronic conditions.
