POOL - VARIABLE - DAYS - WEEKEND REQUIREMENTS: AS NEEDED
One year of related experience.
High School graduate or equivalent.
Under close supervision and according to established procedures, initiates and maintains regular contacts/supports with clients. Assists clients to engage in care by orienting them to the HIV care delivery and financing system; helping them to complete forms if literacy is an issue, accompanying them on visits, assisting a client who has low health literacy, ensuring that transportation is arranged (in coordination with the case manager), assist client with adherence issues, reminding clients of upcoming appointments or needed paperwork, assist client to apply for other benefits (Medicaid, Medicare, etc.), following up with clients regarding missed appointments, and conducting home visits and other activities as needed if a client is lost to care.
