Study inpatient records and determine their authenticity Review patients' documentation to determine diagnosis, co-morbidities, and complications Verify that each code assigned is accurate and correlates to the condition and diagnosis provided in the source document Serve as a resource for coding related queries after verifying authenticity of inquirer Ascertain that quality and integrity of coding including accurate determination of principle diagnosis, secondary diagnosis, and PCS codes Responsible for reviewing patient medical records in the inpatient setting to capture accurate illustration of the severity of illness and facilitate proper coding Facilitate appropriate coding that reflects the severity of the patient's illness along with expected risk of mortality and complexity of care required Documentation of discharge diagnoses and co-morbidities is a comprehensive reflection of the patient's clinical status and care Use advanced knowledge of disease processes (pathophysiology), medications, and have critical thinking skills to analyze current documentation to identify gaps Recognize opportunities in concurrent and retrospective inpatient clinical medical documentation to support quality and effective coding

Graduate with bachelor's degree in nursing 2 to 3 years of work experience in nursing Certification in Clinical Documentation Improvement (CDI) Coding
