The Client Care Service Specialist position will assist in the orientation of every first responder who enters FRW. Responsibilities include maintaining all files documentation, preparation and distribution, arranging meetings, generating reports and emails, client file updates, intake, step-downs. Client Care Service specialists work with internal systems, and our team of mental health professionals for support.
DUTIES & ESSENTIAL JOB FUNCTIONS:
Develop and implement the intake strategy and tactics to create a concierge experience for patients, families, and stakeholders to include program orientation, contact with appropriate parties, intake process via the EMR, and coordination of care through all levels of care.
Work with clinical team members and families upon admission to initiate, document accordingly in the EMR for each client.
Maintain consistent contact with the utilization review team to monitor current and anticipated insurance coverage including level of care changes and DC date.
Assist team and clients with required documentation at intake, discharge, and transitions between all levels of care in between.
Assist internal and external stakeholders with discharge planning by explaining placement options, providing specific referrals based on criteria involved in recommendation, and coordinating the admission for future placement.
Assist clients and families with the identification of a support network, instructions for future care, follow-up and concerns, and requests regarding care upon discharge.
Assist clients with completion of all clinical documentation needed for transition to their continuing care program.
Coordinate and participate in educational and training activities related to continuing care planning for clients.
Assist and support with Alumni events and transition into the alumni programs.
Contact alumni and document accordingly, at the required predetermined points in time, and when needed, after their discharge from the program.
Facilitate and document DC planning groups, family-based groups, and other relevant groups weekly.
Responsible for the overall coordination of Family Week Programming including, but not limited to, sending out invitations, creation of programming packets, ordering food, setting up and breaking down, and all other required on-site activities.
Assist admission department with pre-admission assessments, intakes, admission scheduling and transportation.
Attending all Company staff meetings and training.
This position can also assist with tours and ancillary services associated with the admissions process.
Create, manage, and utilize the Company's discharge referral database and coordinate referrals accordingly with Clinical Outreach.
Manage the company's outcomes program to ensure the tracking of clinical progress, outcomes, and patient/family/stakeholder satisfaction.
Other duties as assigned by supervisor.
Minimum Requirements:
Bachelor's Degree or equivalent experience in Behavioral Health
Current Adult CPR certification
Current Adult First Aid certification
Registered or Certified with a DHCS approved provider
Valid DHCS C3 Form
Valid California Class C Driver's License
Non-Exempt, Monday to Friday or Tuesday to Saturday, Fulltime
Fulltime 40 hours per week, 8 hours per day
