Position Title: Special Education Transition Specialist - Bethel High School

QUALIFICATIONS:

1.Current Connecticut certification as a special education teacher

2.Knowledge of transition services requirements per federal and state regulations.

3.Strong interpersonal skills and ability to form collaborative relationships within the community

4.High degree of initiative and ability to work independently

REPORTS TO: Director of Special Education and Pupil Services and Supervisor of Special Education (6-12+)

JOB GOAL:

The special education transition specialist plans, implements, and evaluates transition related services to students under the Individuals with Disabilities Education Act. This individual will form collaborative relationships within the community in order to provide students with a variety of vocational opportunities as part of the district's Community Partnership Program.

PERFORMANCE RESPONSIBILITIES:

  • Assesses students' interests, skills, and abilities related to post-secondary education, employment, independent living, and community participation using a variety of transitional tools and assessments

  • Plans, implements and evaluates transition related activities for students with disabilities both in the high school and community settings

  • Establishes and maintains an effective community-based training program for students to provide them with relevant and functional experiences.

  • Develops school and community job sites for students' vocational training and development.

  • Provides vocational seminars to students to address work site experiences, problem solves issues, and address vocational, independent living and community participation skills.

  • Serves as a liaison between the district's Planning and Placement Team and the representatives of public and private adult service agencies.

  • Seeks and maintains cooperative relationships with community, regional, and state resources and agencies to support transitioning students and their parents.

  • Provides consultation services to students, parents, and staff in transition-related matters.

  • Establishes school and business partnerships to support students' participation in community-based work experiences.

  • Provides workshops to students, parents, and staff on transition topics and issues, such as information regarding accessing adult service agencies, providers, disability social services, preparing for college and postsecondary education or training, and topics related to students' self-advocacy and self-determination.

  • Plans and coordinates school and community-based situational assessments.

  • Procures guest speakers for students on topics related to transition, transition preparation content, and self-determination.

  • Arranges for a variety of student experiences in accessing the community, including recreation and leisure skills and independent living skills for students preparing to exit high school as well as students in the Community Partnership Program.

  • Develops appropriate transition goals with staff, students, and parents.

  • Plans and coordinates job shadowing experiences for students.

  • Meets regularly with job coaches and paraeducators and provides continuous training, feedback, support, and oversight to them.

  • Coordinates the special education work study and internship program. Monitors and evaluates student performance in the work study program.

  • Continually seeks advanced professional knowledge of the school transition services field, including networking with regional or state transition services team members, attending workshops and state sponsored meetings and trainings related to transition.

  • Serves as a professional resource to high school teachers, pupil services staff, and administrators on matters concerning school to adult transition and the Bethel High School Planning and Placement Team.
