Responsibilities

In alignment with Brookdale's Resident and Family Engagement philosophy, plans, coordinates and facilitates a calendar of dementia friendly programs that meet specific needs of each and all residents within the dementia care community. Provides person centered life enrichment programs while engaging residents in meaningful and purposeful programs within the Brookdale community enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensure resident habits, historical routines and preferences are honored in the provision of care and programming.

Designs a 7 day a week person centered program that is exciting and creative and meets the needs and interests of the individual residents. Promotes and engages residents in the 6 dimensions of Optimum Life purposeful, intellectual, social, spiritual, emotional and physical in a dementia friendly structured environment; including sensory focused small group programming for residents who cannot participate in a large group setting. Ensures life enrichment programs are in compliance with Brookdale Excellence Standards Tool (BEST) standards, state, federal, and other regulations and meet Brookdale's mission, policies, procedures and standards. Leads programs with the residents daily and coordinates the programming assignments of associates, volunteers and entertainment. Coordinates volunteer program. Follows and manages volunteer guidelines. Completes program documentation, as required by Brookdale standards or state requirements Plans, coordinates, appropriately decorates, and participates in special events at the community including holidays, signature programs, family socials, educational and marketing events. Plans and schedules programming events outside the community, which may include driving a community motor vehicle. Acts as a resource for associates and families coping with the changes dementia causes and knowledge of Alzheimer's disease and other dementias. Collaborates in the education of associates regarding dementia care including how to lead programs on an ongoing basis. This includes facilitating dementia focused in-services, computer based and situation specific training. Communicates positively with residents, families, and community professionals regarding Brookdale's Clare Bridge program and its benefits. Communicates with families regarding residents' personal needs and successes. Reports changes in residents' overall health to clinical partner, including changes in behavior; takes actions to address concerns in a timely manner and collaborates with the team in identifying interventions to address behavioral expressions. Participates, maintains and monitors department quality improvement tools including the (BEST), budget and supplies as assigned by the Executive Director. Supports and partners with leadership in organizing the National Walk to End Alzheimer'steam in the community or local area. Performs personal care, housekeeping and dietary responsibilities, as needed, to ensure service quality is maintained in accordance with Clare Bridge Program standards. Has completed training and qualifications based on state regulations to ensure service quality is maintained in accordance with Clare Bridge Program standards. May directly supervise Clare Bridge Program Assistants; may act in supervisory capacity in absence of Manager or Executive Director. Assists Manager/Director/ED with hiring, training, and evaluating Clare Bridge Program Assistants.

Qualifications

Education and ExperienceBachelor's Degree in therapeutic recreation, gerontology, education or related field preferred. Minimum two years of direct programming experience with older adults and persons with dementia preferred; or equivalent combination of education and experience.

Certifications, Licenses, and Other Special RequirementsMust have a valid driver's license and a current 3-year driving abstract with no more than one moving violation as a condition of employment.

Management/Decision MakingUses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.

Knowledge and SkillsHas a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization. Basic typing skills are essential along with basic knowledge of PC's and word processing software, preferably in the Microsoft Windows environment.

Responsibilities Provides direct supervision of care giving within the community. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community. Provides direct supervision of resident care staff, which includes monitoring job performance, conducting associate performance evaluations, coaching and counseling associates and conducting and documenting corrective action as needed. Coordinates training and education of resident care staff on an ongoing basis. Verifies all associate hours on time cards and weekly hour's report. Completes nursing schedules to assure proper coverage to meet resident needs and according to budget. Assigns weekly blood pressures and weights to rotating shifts. Monitors staff in administration of medication, to include frequents audits of medication sheets. Assists the Health and Wellness Director in medication management, including administration of medications to residents, ordering medication from pharmacy and maintaining records as dictated in Brookdale Senior Living policies and procedures manual. Assures that medications are properly stored, medication carts/cupboards are cleaned and medication room is organized according to standards. Observes residents health status, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment or significant incidences to Health and Wellness Director in a timely manner. Reviews capabilities of resident who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation). Conducts pre-admission screening of prospective new residents when assigned by supervisor and updates every six months or as needed. Assures that all required documentation, including service plans and nursing assessments, is completed. Performs ongoing assessments of resident physical and psycho-social needs and coordinates with nurse to assure quality proactive care as allowed by state regulation. Communicates to families regarding resident's personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments. Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families. Observes residents, records significant reactions, and notifies Executive Director, Health and Wellness Director, physician, and pharmacist (as applicable) of resident's condition and reaction to drugs, treatments, and significant incidents. Responds promptly to all emergency calls from residents, including residents residing in independent section of community. Performs daily, weekly and monthly safety and infection control checks of the home and grounds, assisting the Executive Director and the Health and Wellness Director for resident health and safetypurposes. Coordinates monthly staff schedules, arranges replacement staffing when necessary and maintains clear records of all changes, requests and irregularities. Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are scheduled and followed through. Participates in the maintenance of resident records including review of documentation performed by resident assistants, monthly MAR reviews, setting up new resident and thinning records appropriately. Participates in department quality improvement activities, to include completion of quality assurance surveys as assigned by Executive Director and/or the Health and Wellness Director. Shares on call duties as required. Participates in community functions including family socials and open houses. Communicates with community professionals.
